摘 要:目的探究模拟教学在普外科住院医师规范化培训中的应用。方法纳入上海交通大学医学院附属瑞金医院卢湾分院普外科于2017年1月—2020年12月接收的80名住院医师作为研究样本,采用随机数字表法分为两组,各40名,观察组给予情景模拟教学法(scene-simulated learning,SSL),对照组给予传统教学模式,评价两组考核成绩、迷你临床演练评估(mini clinical evaluation exercise,Mini CEX)评分、教学效果、满意度。结果观察组理论成绩、技能成绩均低于对照组,差异有统计学意义(t=16.421、16.005,P<0.05)。观察组住院医师医学面谈技能、查体技能、人文素质/专业精神、临床判断、沟通技巧、条理性/效率、临床综合能力评分均高于对照组,差异有统计学意义(P<0.05)。观察组住院医师满意度87.50%,高于对照组67.50%,差异有统计学意义(P<0.05)。观察组提高自主学习能力、提高学习兴趣、提高临床思维能力、提高沟通交流能力、提高团队合作精神占比均高于对照组,差异有统计学意义(P<0.05)。结论普外科住院医师规范化培训中应用模拟教学法可提高理论与技能考核成绩,提高临床综合能力,住院医师对教学效果与教学满意度评价高。Objective To explore the application of simulation teaching in the standardized training of general surgery residents.Methods The 80 resident physicians accepted by the Luwan branch of the Ruijin hospital of Shanghai Jiaotong university school of medicine between January 2017 and December 2020 were included as the research samples.Using random number table method,they were divided into two groups with 40 residents in each group.The observation group was given scene-simulated learning(SSL),and the control group was given the traditional teaching mode.Evaluated the two groups of assessment results,mini clinical evaluation exercise(Mini CEX)score,teaching effect,and satisfaction.Results The theoretical performance and skill performance of the observation group were lower than those of the control group,the difference was statistically significant(t=16.421,16.005,P<0.05).The scores of medical interview skills,physical examination skills,humanistic quality/professionalism,clinical judgment,communication skills,organization/efficiency,and clinical comprehensive ability of the observation group were higher than those of the control group,the difference was statistically significant(P<0.05).The satisfaction degree of resident in the observation group was 87.50%,which was higher than 67.50%in the control group,the difference was statistically significant(P<0.05).The proportion of the observation group's improvement in independent learning ability,increasing interest in learning,improving clinical thinking ability,improving communication skills,and improving teamwork spirit were all higher than those in the control group,the difference was statistically significant(P<0.05).Conclusion The application of simulated teaching method in the standardized training of general surgery residents can improve the performance of theory and skills assessment,and improve the clinical comprehensive ability.Residents have high evaluation of the teaching effect and teaching satisfaction.
