高分辨力CT对曲菌球型肺曲霉菌病的诊断价值

摘　　要：目的探究高分辨力CT(HRCT)对曲菌球型肺曲霉菌病的诊断价值。方法选取医院2011年8月至2018年6月收治的经手术病理证实的16例曲菌球型肺曲霉菌病患者,术前均行X线及HRCT检查,比较X线与HRCT对曲菌球型肺曲霉菌病的检出率,并观察曲菌球型肺曲霉菌病患者的影像特征。结果HRCT对曲菌球型肺曲霉菌病的检出率(100.0%)高于X线(12.5%),差异有统计学意义(P<0.05)。经HRCT扫描,曲菌球型肺曲霉菌病患者表现为空洞或空腔性病变内球形内容物,即菌球,菌球表现为洞腔内软组织样密度,其中菌球呈类圆形或椭圆形14例、不规则软组织块2例,菌球边缘光滑14例、毛糙2例,钙化1例;14例患者空洞或空腔壁与菌球之间可见新月形或环形透亮影,改变体位菌球位置可发生变化。X线检查大多表现不典型,检出的2例患者,菌球呈结节状高密度影。结论相较于X线,HRCT对曲菌球型肺曲霉菌病的检出率较高,且影像特征较典型。Objective The diagnostic value of high resolution CT(HRCT)in aspergilloma type of pulmonary aspergillosis was explored.Methods 16 patients with aspergilloma pulmonary aspergillosis confirmed by surgery and pathology received in the hospital from August 2011 to June 2018 were selected.X-ray and HRCT were performed before operation.The detection rates of X-ray and HRCT were compared,and the HRCT imaging features of patients with aspergilloma pulmonary aspergillosis were observed.Results The detection rate of aspergilloma type of pulmonary aspergillosis by HRCT was 100.00%,which was higher than that by X-ray(12.5%),and the difference was statistically significant(P<0.05).HRCT scan results showed that the imaging features of patients with aspergilloma type of pulmonary aspergillosis appeared spherical contents in cavitary lesions.The spherical contents which was also called bacteria sphere appeared as density opacity like soft tissue in cavity.The bacteria sphere of 14 cases were quasi-circular or oval,while the other 2 cases presented anomalous soft tissue.The edges of bacteria sphere of 14 cases were smooth,while the other 2 cases were coarse,and 1 case presented calcific.Annulus and crescent bright sign was found between cavity wall and bacteria sphere in 14 patients,and the position of the bacteria sphere changed with posture changing.The imaging features of X-ray were not typical in sum,for the 2 cases which was detected by it,their bacteria sphere presented nodular high-density opacity.Conclusion Compared with X-ray,the detection rate of aspergilloma type of pulmonary aspergillosis by HRCT is higher and meanwhile its imaging features are relatively more typical.
