健脾助运清泄方治疗小儿幽门螺旋杆菌相关性胃病临床疗效及对血清Hsp60、VacA水平的影响

摘　　要：目的:探究健脾助运清泄方治疗小儿幽门螺旋杆菌(Hp)相关性胃病对血清热休克蛋白60(Hsp60)、空泡形成毒素(VacA)水平的影响。方法:将2019年1月~2020年6月期间于我院就诊的74例Hp相关性胃病患儿随机分为观察组37例和对照组37例,对照组实施常规西药三联用药方案,观察组在对照组基础上予以健脾助运清泄方治疗,比较治疗3w后两组中医证候疗效差异,比较治疗前及治疗3周后两组中医症状积分、Hp毒力因子(Hsp60、VacA)、炎性介质[白细胞介素-17(IL-17)、白细胞介素-8(IL-8)、可溶性白细胞介素-2受体(slL-2R)、化生长因子-β(TGF-β)]、细胞免疫功能(CD_(4)^(+)、CD_(8)^(+)、CD_(4)^(+)/CD_(8)^(+))变化。结果:治疗3周后,观察组总有效率显著高于对照组(P<0.05);两组中医症状积分、Hsp60、VacA、IL-17、IL-8、slL-2R、CD_(8)^(+)水平均较治疗前显著降低,且观察组明显低于同一时间对照组(P<0.05);两组TGF-β、CD_(4)^(+)、CD_(4)^(+)/CD_(8)^(+)水平均较治疗前显著升高,且观察组显著高于对照组(P<0.05)。结论:健脾助运清泄方治疗小儿Hp相关性胃病疗效较佳,可有效抑制患儿Hp毒性反应、炎症反应,亦利于改善其细胞免疫功能,有较高临床应用价值。Objective:To explore the effect of Jianpi Zhuyun Qingxie Decoction on levels of serum heat shock protein 60(Hsp60)and vacuolating cytotoxin(VacA)for the treatment of Helicobacter pylori(Hp)-associated gastropathy in children.Methods:A total of 74 children patients with Hp-associated gastropathy who were treated in our hospital during the period from January 2019 to June 2020 were randomly divided into an observation group(37 cases)and a controlled group(37 cases).The controlled group was given conventional western medicine triple-drug regimen,and the observation group was given Jianpi Zhuyun Qingxie Decoction on the basis of the controlled group.The efficacy of TCM syndromes was compared between the two groups after 3 weeks of treatment.The scores of TCM symptoms,Hp virulence factors(Hsp60,VacA),inflammatory mediators[interleukin-17(IL-17),interleukin-8(IL-8),soluble interleukin-2 receptor(slL-2 R),transforming growth factor-β(TGF-β)]and cellular immune function(CD_(4)^(+),CD_(8)^(+),CD_(4)^(+)/CD_(8)^(+))were compared between the two groups before treatment and after 3 weeks of treatment.Results:After 3 weeks of treatment,the total effective rate of the observation group was significantly higher than that of the controlled group(P<0.05).The scores of TCM symptoms and levels of Hsp60,VacA,IL-17,IL-8,slL-2 R and CD_(8)^(+)of the two groups were significantly decreased compared with those before treatment,and the scores and levels of the observation group were significantly lower than those of the controlled group at the same time point(P<0.05).The levels of TGF-β,CD_(4)^(+)and CD_(4)^(+)/CD_(8)^(+)in the two groups were significantly increased compared with those before treatment,and the levels of the observation group were significantly higher than those of the controlled group(P<0.05).Conclusion:Jianpi Zhuyun Qingxie Decoction has a good efficacy on Hp-associated gastropathy in children.And it can effectively inhibit Hp toxic response and inflammatory reactions,and it is also beneficial to improve the cellula
