摘  要:目的总结创伤后患者并发吉兰-巴雷综合征的神经电生理特点,为早期诊断与治疗提供依据。方法选取本院骨科、胸外科、ICU及神经内科创伤后发生四肢无力和(或)呼吸困难,符合吉兰-巴雷综合征的诊断标准的患者12例,对其进行神经传导检测、F波、H反射检测及针极肌电图。结果 12例患者均表现为多发周围神经损害(运动神经受累为主,轴索损害显著)。其中运动神经异常率为66.07%,感觉神经异常率为24.37%,F波异常率为74.63%,H反射异常率为100%。针极肌电图仅1例可见自发电位,12例均表现为主动募集反应减弱或无力。结论创伤相关性吉兰-巴雷综合征的神经电生理特点主要为运动神经轴索损害为主,神经电生理检测可对其进行早期诊断,并能鉴别与吉兰-巴雷综合征相似的危重症性肌病及多发性神经病。Objective To summarize the neuroelectrophysiological characteristics of post-traumatic patients with Guillain Barré syndrome,and to provide basis for early diagnosis and treatment. Methods Twelve patients with limb weakness,and/or dyspnea after trauma in orthopaedics,thoracic surgery,ICU and neurology department of our hospital were selected to meet the diagnostic criteria of Guillain-Barré syndrome. Nerve conduction test,F wave,H reflex and electromyography were performed. Results All 12 patients showed multiple peripheral nerve damage(mainly motor nerve involvement,axonal damage was significant).Among them,the abnormal rate of motor nerve,sensory nerve,F wave and H reflex were 66.07%,24.37%,74.63% and 100%,respectively. Only 1 case showed spontaneous potential,and 12 cases showed weakening or weakening of active recruitment response.Conclusion The neuroelectrophysiological characteristics of trauma related Guillain Barré syndrome are mainly motor axonal damage. The neuroelectrophysiological detection can be used for early diagnosis,and can identify the critical myopathy and polyneuropathy similar to Guillain Barré syndrome.
