安氏Ⅲ类错[牙合]不同垂直骨面型患者颞下颌关节间隙的CBCT比较分析

摘　　要：目的:对安氏Ⅲ类错[牙合]不同垂直骨面型颞下颌关节间隙进行CBCT比较分析。方法:对错[牙合]高角,低角患者和正常人各30例进行CBCT测量并比较。结果:高角组的髁突长轴角度髁突倾角比对照组大,髁突高度和长轴直径小、相对颅底靠后、关节后间隙大、关节上间隙小、关节窝浅,关节结节后斜面角小(P<0.05)。低角组倾角较对照组小,长轴直径大,颅底位置靠下,关节后间隙小,前间隙大(P<0.05)。低角组髁突后移位者比例较大,高角组前移位者比例较大。结论:安氏Ⅲ类低角患者、高角患者和正常人群之间关节窝、髁突形态和位置均存在一定程度的不同。Objective:To compare the TMJ space among the different vertical bone face types in the subjects with angle classⅢmalocclusion.Methods:30 cases with high angle and 30 cases with low angle of the angle classⅢsubjects were included.Another 30 subjects with normal occlusion were served as the controls.The TMJ space of all subjects were measured by CBCT technology.Results:The angle of condyle long axis in high angle group was larger than that in the control group,while the height and diameter of condyle long axis were smaller.The condyle in high angle group was more backward,the postarticular space was larger,the supraarticular space was smaller,the articular fossa was shallower,and the angle of posterior oblique plane of articular node was smaller,all of which had significant difference(P<0.05).In the low angle group the angle of comdyle long axis was smaller,while the long axis diameter was larger.The skull base in low angle group was lower than that in the control group.The posterior joint space was smaller,but the anterior joint space was larger(P<0.05).The ratio of posterior displacement in low angle group was larger,that of anterior displacement in high angle group.Conclusion:The shape and position of the fossa and condyle of the patients with angleⅢmalocclusion are different from those of the subjects with normal occlusion and there are some differences between the subjects with high and lower angle.
