胃镜下重酒石酸去甲肾上腺素注射治疗急性非静脉曲张性上消化道出血临床效果研究

摘　　要：目的分析胃镜下重酒石酸去甲肾上腺素注射治疗急性非静脉曲张性上消化道出血(ANVUGIB)的临床效果。方法选取2018年3月—2020年3月本院收治的89例ANVUGIB,按照治疗方式的差异分为观察组49例(予胃镜下重酒石酸去甲肾上腺素注射治疗)和对照组40例(予胃镜下重酒石酸去甲肾上腺素喷洒治疗)。比较2组临床疗效,住院情况(住院时间、输血量、止血时间),不良反应(大便频繁、胀气、血压不稳、腹部不适)及预后(再出血率、外科手术率)。结果观察组临床疗效明显高于对照组(P<0.01);观察组住院时间、止血时间明显短于对照组,输血量少于对照组,差异有统计学意义(P<0.01);观察组不良反应发生率低于对照组(P<0.01);治疗后1周观察组再出血率及外科手术率明显低于对照组(P<0.05,P<0.01)。结论胃镜下重酒石酸去甲肾上腺素注射治疗ANVUGIB效果确切,可有效缩短患者止血时间及住院时间,减少输血量,不良反应发生率低,且预后佳。Objective To analyze the clinical efficacy of Norepinephrine Bitartrate Injection under gastroscope in treatment of patients with acute non-variceal upper gastrointestinal bleeding(ANVUGIB).Methods A total of 89 patients with ANVUGIB admitted between March 2018 and March 2020 were divided into observation group(treated with Norepinephrine Bitartrate Injection under gastroscope,n=49)and control group(treated with Norepinephrine Bitartrate spraying under gastroscope,n=40).Clinical efficacy,hospitalization conditions(length of stay,blood transfusion volume,duration of hemostasis),adverse reactions(frequency of defecation,flatulence,unstable blood pressure,abdominal discomfort)and prognoses(rebleeding rate,surgical operation rate)were compared between two groups.Results Compared with those in control group,in observation group,clinical efficacy was significantly higher(P<0.01);length of stay and duration of hemostasis were significantly shorter,and the volume of blood transfusion was less(P<0.01);incidence rate of adverse reactions was lower(P<0.01);after treatment for one week,the rebleeding rate and surgical operation rate were significantly lower(P<0.05,P<0.01).Conclusion Norepinephrine Bitartrate Injection under gastroscope is effective in treatment of ANVUGIB.It may effectively shorten duration of hemostasis and length of stay,reduce the volume of blood transfusion and incidence rate of adverse reactions with good prognosis.
