外周血IgG4、IgG及IgG4/IgG比值与Graves眼病严重程度、甲状腺免疫抗体的相关性分析

摘　　要：目的探讨外周血免疫球蛋白G4(IgG4)、IgG及IgG4/IgG比值与Graves眼病严重程度、甲状腺免疫抗体的相关性。方法选取2018年1月—2020年12月收治的Graves眼病105例,根据疾病严重程度分为轻度组49例、中重度组40例、极重度组16例。选取同期健康体检者45例为对照组。检测血清IgG4、IgG以及抗甲状腺球蛋白抗体(TGAb)、抗甲状腺过氧化物酶抗体(TPOAb)、抗促甲状腺素受体抗体(TRAb)水平,分析血清IgG4与甲状腺免疫抗体的相关性。结果与对照组比较,轻度、中重度、极重度组血清IgG4水平、IgG4/IgG比值增高,且随着疾病严重程度升高而增高(P<0.01)。与对照组比较,Graves眼病轻度组、中重度组、极重度组TGAb、TPOAb、TRAb阳性检出率均增高(P<0.01)。血清IgG4与Graves眼病严重程度、TGAb阳性、TPOAb阳性、TRAb阳性呈正相关(r=0.616、0.599、0.338、0.229,P<0.01)。结论外周血IgG4与Graves眼病严重程度、甲状腺免疫抗体具有相关性,可为病情评估提供参考。Objective To investigate correlations between peripheral blood immunoglobulin G4(IgG4),IgG and IgG4/IgG ratio with the severity of Graves’ophthalmopathy and thyroid immune antibodies.Methods A total of 105 patients with Graves’ophthalmopathy admitted between January 2018 and December 2020 were selected and divided into mild group(n=49),moderate-severe group(n=40)and extremely severe group(n=16),and other 45 healthy individuals who had undergone physical examinations at the same period were selected as healthy control group.Serum levels of IgG4,IgG,anti-thyroglobulin antibody(TGAb),anti-thyroid peroxidase antibody(TPOAb)and anti-thyrotropin receptor antibody(TRAb)were detected,and correlations between serum IgG4 with thyroid immune antibodies were analyzed.Results Serum IgG4 levels and IgG4/IgG ratios were significantly higher in mild,moderate-severe and extremely severe groups than those in control group,and the level and ratio were increased with increasing severity of the disease(P<0.01).The positive detection rates of TGAb,TPOAb and TRAb in mild,moderate-severe and extremely severe groups were significantly higher than those in control group(P<0.01),but there were not statistically significant differences in the positive detection rates among mild,moderate-severe and extremely severe groups(P>0.05).Serum IgG4 was positively correlated with the severity of Graves’ophthalmopathy,positive TGAb,positive TPOAb and positive TRAb(r=0.616,0.599,0.338,0.229,P<0.01).Conclusion Peripheral blood IgG4 is correlated with the severity of Graves’ophthalmopathy and thyroid immune antibodies,which may provide reference for evaluation of the patient’s condition.
