两颗种植体支持的采用舌侧集中[牙合]型的下颌覆盖义齿对种植体的影响

摘　　要：目的:探讨两颗种植体支持的采用舌侧集中[牙合]型的下颌覆盖义齿对种植体的影响。方法:将2018年6月-2019年10月本院收治的41例完全无牙患者按照随机数字表法分为两组,对照组20例佩戴种植体支持的采用解剖式双侧平衡[牙合]型的下颌覆盖义齿,观察组21例佩戴种植体支持的采用舌侧集中[牙合]型的下颌覆盖义齿。观察两组植体深度和边缘骨丧失量、咀嚼效率和咬合力、功能障碍指数和颞下颌关节指数。结果:佩戴3、6、12个月后,观察组植体深度均深于对照组,边缘骨丧失量均小于对照组,差异均有统计学意义(P<0.05)。佩戴3、6个月后,观察组咀嚼效率和咬合力均高于对照组,功能障碍指数和颞下颌关节指数均低于对照组,差异均有统计学意义(P<0.05)。结论:两颗种植体支持的采用舌侧集中[牙合]型的下颌覆盖义齿可以提高义齿的咀嚼效率和咬合力,降低颌关节指数与功能障碍指数,进而加深种植体植体深度,减少边缘骨丧失量。Objective:To explore the influence of mandibular overdenture supported by two implants with lingual concentrated occlusion on implants.Method:Forty-one completely edentulous patients admitted to our hospital from June 2018 to October 2019 were randomly divided into two groups.The control group(20 cases)wore implant-supported mandibular overdentures with anatomical bilateral balanced occlusion type,and the observation group(21 cases)wore implant-supported mandibular overdentures with lingual concentrated occlusion type.The implant depth,marginal bone loss,masticatory efficiency and bite force,dysfunction index and temporomandibular joint index of two groups were observed.Result:After wearing for 3,6,12 months,the implant depth of the observation group were deeper than those of the control group,the loss of marginal bone were less than those of control group,the differences were statistically significant(P<0.05).After wearing for 3,6 months,the masticatory efficiency and bite force of the observation group were higher than those of the control group,and the dysfunction index and temporomandibular joint index were lower than those of the control group,the differences were statistically significant(P<0.05).Conclusion:The mandibular overdenture supported by two implants with lingual concentrated occlusion type can easily improve the masticatory efficiency and bite force,reduce the jaw joint index and dysfunction index,and then deepen the implant depth and reduce the loss of marginal bone.
