分析盐酸米诺环素软膏治疗固定正畸牙龈炎的临床效果

摘　　要：目的评价盐酸米诺环素软膏治疗固定正畸牙龈炎患者的临床效果。方法144例固定正畸牙龈炎患者,应用数字双盲法分为参照组与实验组,每组72例。参照组采取常规口腔卫生保健措施进行治疗,实验组采取盐酸米诺环素软膏进行治疗。比较两组患者临床效果及治疗前后牙周功能指标、炎性介质、疼痛介质水平。结果实验组患者治疗总有效率为97.22%,高于参照组的86.11%,差异具有统计学意义(P<0.05)。治疗后,实验组患者龈沟出血指数(SBI)、探诊深度(PD)、牙龈指数(GI)分别为(1.17±0.43)分、(2.75±0.32)mm、(1.38±0.65)分,均低于参照组的(1.31±0.41)分、(3.01±0.33)mm、(1.61±0.66)分,差异具有统计学意义(P<0.05)。治疗后,实验组患者白细胞介素-1β(IL-1β)、基质金属蛋白酶-8(MMP-8)以及前列腺素E2(PGE2)水平低于参照组,差异具有统计学意义(P<0.05)。结论在固定正畸牙龈炎的治疗中,盐酸米诺环素软膏效果更为显著,可改善牙周功能,降低疼痛介质与炎性介质水平,具有临床应用价值。Objective To evaluate the clinical effect of minocycline hydrochloride ointment in the treatment of gingivitis during fixed orthodontic treatment.Methods A total of 144 patients with gingivitis during fixed orthodontic treatment were divided into reference group and experimental group by digital doubleblind method,with 72 cases in each group.The reference group was treated with conventional oral health care measures,and the experimental group was treated with minocycline hydrochloride ointment.The clinical effects,periodontal function indexes,inflammatory mediators,and pain media levels before and after treatment were compared between the two groups.Results The total effective rate of treatment in the experimental group was 97.22%,which was higher than 86.11%in the reference group,and the difference was statistically significant(P<0.05).After treatment,the gingival sulcus bleeding index(SBI),probing depth(PD),and gingival index(GI)of the experimental group were(1.17±0.43)points,(2.75±0.32)mm,and(1.38±0.65)points,respectively,which were lower than(1.31±0.41)points,(3.01±0.33)mm,and(1.61±0.66)points of the reference group,and the difference was statistically significant(P<0.05).After treatment,the levels of interleukin-1β(IL-1β),matrix metalloproteinase-8(MMP-8)and prostaglandin E2(PGE2)in the experimental group were lower than those in the reference group,and the difference was statistically significant(P<0.05).Conclusion Minocycline hydrochloride ointment has remarkable effect on gingivitis during fixed orthodontic treatment,and it can improve periodontal function,reduce the level of pain mediators and inflammatory mediators,which contains the value of clinical application.
