云南白药胃管反复给药治疗急性上消化道出血的临床有效性

摘　　要：目的:分析急性上消化道出血患者云南白药胃管反复给药治疗的临床有效率。方法:以笔者所在医院急诊科2018年6月-2020年6月收治的102例急性上消化道出血患者为研究主体,采用随机数字表法划平均分为两组。常规组51例采取胃肠保护、卧床休息、抑制胃酸等常规治疗,试验组51例在此基础上采取云南白药胃管反复给药治疗,分析两组治疗效果、血清炎性因子水平、治疗情况、不良反应等。结果:试验组治疗总有效率显著高于常规组(P<0.05);试验组胃液pH值及血红蛋白水平高于常规组,止血时间、大便转黄时间早于常规组,输血量少于常规组,住院时间短于常规组(P<0.05);试验组治疗后的TNF-α、IL-8、IL-6水平低于常规组(P<0.05);试验组不良反应发生率低于常规组(P<0.05)。结论:在急性上消化道出血的治疗中采用云南白药胃管反复给药具有显著效果,可有效提高治疗效果,显著改善症状,快速止血,降低胃液pH值、血清炎性因子水平等指标,且此种治疗方式不良反应少,值得应用。Objective:To analyze the clinical efficiency of Yunnan Baiyao repeated administration through gastric tube in patients with acute upper gastrointestinal bleeding.Method:A total of 102 patients with acute upper gastrointestinal bleeding admitted to the emergency department in our hospital from June 2018 to June 2020 were selected as the research subjects,and they were randomly divided into two groups with equal number of cases.Fifty-one cases in the conventional group were treated with routine treatment such as gastrointestinal protection,bed rest,inhibition of gastric acid,etc.Fifty-one cases in the experimental group were treated with repeated administration of Yunnan Baiyao through gastric tube.The therapeutic effect,serum inflammatory factor level,treatment situation and adverse reactions of the two groups were analyzed.Result:The total effective rate of the experimental group was significantly higher than that of the conventional group(P<0.05);the pH value of gastric juice and hemoglobin level of the experimental group were higher than those of the conventional group,the hemostasis time and stool turning yellow time of the experimental group were earlier than those of the conventional group,the amount of blood transfusion was less than that of the conventional group,and the hospitalization time was shorter than that of the conventional group(P<0.05);the levels of TNF-α,IL-8 and IL-6 of the experimental group after treatment were lower than those of the conventional group(P<0.05);the incidence of adverse reactions in the experimental group was lower than that in the conventional group(P<0.05).Conclusion:In the treatment of acute upper gastrointestinal bleeding,repeated administration of Yunnan Baiyao through gastric tube has a significant effect,which can effectively improve the treatment effect,significantly improve the symptoms of the disease,can quickly stop bleeding,reduce the pH value of gastric juice,serum inflammatory factor levels and other indicators,and has less adverse reactions.It is worthy of app
