紧急内镜检查在急性上消化道出血患者中的应用效果

摘　　要：目的:研究紧急内镜检查在急性上消化道出血患者中的应用效果。方法:回顾性分析本院2019年1月-2020年12月收治的100例急性上消化道出血患者的临床资料,依据内镜检查时机分为紧急内镜组及早期内镜组,各50例,紧急内镜组于就诊后6 h内实施内镜检查,早期内镜组于就诊后6~24 h实施内镜检查,对比两组30 d死亡率、30 d内再出血率、活动性出血及血管溃疡诊断率差异。结果:紧急内镜组30 d死亡率、30 d内再出血率分别为8.00%、12.00%,略高于早期内镜组的6.00%、10.00%,差异均无统计学意义(P>0.05);以手术结果为参照,紧急内镜组活动性出血诊断率明显高于早期内镜组,差异有统计学意义(P<0.05)。结论:紧急内镜检查能够进一步提高急性上消化道出血患者诊断结果可靠性但与30 d死亡率、30 d内再出血率无关。Objective:To study the application effect of emergency endoscopy in patients with acute upper gastrointestinal bleeding.Method:A retrospective analysis of the clinical data of 100 patients with acute upper gastrointestinal bleeding admitted to our hospital from January 2019 to December 2020.According to the timing of endoscopy,they were divided into emergency endoscopy group and early endoscopy group,50 cases in each group,in the emergency endoscopy group,endoscopy was performed within 6 hours after the consultation,and the early endoscopy group was performed endoscopy between 6-24 hours after the consultation.The 30-day mortality,30-day rebleeding rate,diagnosis rate of active bleeding and vascular ulcer were compared between the two groups.Result:The 30-day mortality rate and the 30-day rebleeding rate in the emergency endoscopy group were 8.00%and 12.00%respectively,which were slightly higher than 6.00%and 10.00%in the early endoscopy group,there were no significant differences(P>0.05);with the surgical results as the reference,the diagnostic rate of active bleeding in emergency endoscopy group was significantly higher than that in early endoscopy group,and the differences were statistically significant(P<0.05).Conclusion:Emergency endoscopy can further improve the reliability of the diagnosis of patients with acute upper gastrointestinal bleeding,but it has nothing to do with the 30-day mortality and the 30-day rebleeding rate.
