摘 要:目的:分析糖尿病患者口腔颌面部多间隙感染的临床特征。方法:将2018年5月-2020年5月笔者所在医院收治的71例患有糖尿病的口腔颌面部多间隙感染患者设为糖尿病组,并选取同期笔者所在医院收治的无糖尿病的71例口腔颌面部多间隙感染患者作为非糖尿病组。比较两组口腔颌面部多间隙感染的临床特征。结果:牙源性感染是导致两组口腔颌面部多间隙感染的主要病因,两组感染病因比较差异无统计学意义(P=0.05)。糖尿病组中受累2个以上间隙比例大于非糖尿病组,差异有统计学意义(P<0.05)。糖尿病组并发症发生率明显高于非糖尿病组,差异有统计学意义(P<0.001)。糖尿病组肺炎克雷伯菌检出率明显高于非糖尿病组,差异有统计学意义(P<0.05),两组其他细菌检出率比较差异无统计学意义(P>0.05)。结论:相比于非糖尿病患者,糖尿病患者口腔颌面部多间隙感染的受累间隙数量更多,肺炎克雷伯菌检出率较高,并发症发生率更高,因此,对患者的血糖进行控制可有效避免患者出现并发症的风险。Objective:To analyze the clinical characteristics of oral and maxillofacial multiple space infection in diabetes patients.Method:A total of 71 diabetes patients with oral and maxillofacial multiple space infection admitted in our hospital from May 2018 to May 2020 were set as the diabetes group,and 71 non-diabetes patients with oral and maxillofacial multiple space infection admitted in our hospital during the same period were set as the non-diabetes group.The clinical characteristics of oral and maxillofacial multiple space infection of the two groups were compared.Result:Odontogenic infection was the main cause of oral and maxillofacial multiple space infection in both groups.There was no significant difference in the causes of infection between the two groups(P=0.05).The proportion of more than 2 spaces involved in the diabetes group was higher than that in the non-diabetes group,and the difference was statistically significant(P<0.05).The incidence of complications in the diabetes group was significantly higher than that in the non-diabetes group(P<0.001).The detection rate of Klebsiella pneumoniae in the diabetes group was significantly higher than that in the non-diabetes group,the difference was statistically significant(P<0.05),while there was no significant difference in the detection rate of other bacteria in the two group(P>0.05).Conclusion:Compared with non-diabetes patients,diabetes patients have more affected spaces of oral and maxillofacial multiple space infection,higher detection rate of Klebsiella pneumoniae and higher incidence of complications.Therefore,blood glucose control can effectively avoid the risk of complications in patients.
