摘 要:目的:探讨脱抑制训练对单眼性远视性屈光参差性弱视患儿屈光参差程度的影响。方法:前瞻性病例对照研究。2016年3月-2019年1月将本院50例单眼性远视性屈光参差性弱视患儿随机分为脱抑制训练组(26例26眼)和对照组(24例24眼)。对照组采用弱视治疗仪结合网络软件相关精细刺激训练;脱抑制训练组在对照组基础上,当两眼视力相差两行以内时进行脱抑制训练。分别于训练3、6、9个月及1年后测量眼轴及综合验光,计算健眼与弱视眼眼轴差值及屈光度差值,并进行统计学分析。结果:训练前脱抑制训练组健眼与弱视眼眼轴差值较对照组高约0.3 mm,差异有统计学意义(t=2.464,P<0.05),训练1年后低于对照组约0.4 mm(t=3.497,P<0.05);训练前脱抑制训练组健眼与弱视屈光度差值高于对照组约0.73 D,差异有统计学意义(t=2.237,P<0.05),训练1年后低于对照组约1.00 D,差异有统计学意义(t=3.280,P<0.05)。结论:单眼性远视性屈光参差性弱视患儿在治疗过程中,加入脱抑制训练解除大脑对弱视眼的抑制及增强弱视眼的视觉竞争力,可促进弱视眼的眼球发育,使双眼眼轴差值及屈光度差值明显降低,有效降低屈光参差程度。Objective:To explore the effect of disinhibition training on the degree of anisometropia in children with monocular hyperopic anisometropic amblyopia.Method:Prospective case-control study.Fifty children with monocular hyperopic anisometropic amblyopia were randomly divided into 2 groups from March 2016 to January 2019:disinhibition training group(26 cases,26 eyes)and control group(24 cases,24 eyes).The control group was trained as follows:amblyopia therapeutic apparatus,combined with network software and adjustment training.The deinhibition training was performed when the visual acuity difference between the two eyes was within two lines on the basis of control group.The eye axis and comprehensive optometry were measured after 3,6,9 months and 1 year of training.The axial and diopter differences between healthy and amblyopic eyes were calculated and statistically analyzed.Result:The axial difference between healthy and amblyopic eyes in disinhibition training group was about 0.3 mm higher than control group before training,which was statistically significant(t=2.464,P<0.05),and it was 0.4 mm lower than control group after 1 year(t=3.497,P<0.05);the diopter difference between healthy eyes and amblyopia in disinhibition training group was about 0.73 D higher than control group before training,which was statistically significant(t=2.237,P<0.05),and it was 1.00 D lower than control group after 1 year,which was statistically significant(t=3.280,P<0.05).Conclusion:In the treatment of children with monocular hyperopia anisometropic amblyopia,disinhibition training is added to relieve the inhibition of brain on amblyopia and enhance the visual competitiveness of amblyopia,which can promote the eyeball development of amblyopia,significantly reduce the axial difference and diopter difference between eyes,and effectively reduce the degree of anisometropia.
