牟允方运用附子治疗危重症验案举隅

摘　　要：[目的]总结浙江东南地区名老中医牟允方运用附子治疗危重症的临证经验。[方法]通过整理、研究牟允方老中医的临床病案,选取误汗亡阳、吐血、麻毒闭肺、胆石症4则危重症验案,分析其运用附子配伍使用治疗危重症的理论及独到经验。[结果]牟允方老中医在临床危重症治疗中擅长阴阳寒热辨证,灵活巧妙运用附子配伍,取得较好疗效。如误汗亡阳案为阴寒内盛的格阳证,治以附子配干姜的回阳法;吐血案证属气随血脱、阴损及阳,治以附子配人参的温补法;麻毒闭肺案为邪毒内闭、阳气外脱,治以附子配石膏的温清法;胆石症为少阳与阳明合病,兼有脾虚寒湿,故用附子配大黄的温下法。[结论]牟允方老中医运用附子配伍治疗危重症的辨治经验,思路独到,疗效显著,值得学习和借鉴。[Objective]To summarize the clinical experience of MOU Yunfang,a famous old TCM doctor in southeast of Zhejiang,in treating critical disease with Aconiti Lateralis Radix Preparata.[Methods]Through sorting out and studying the clinical medical records of MOU Yunfang,this paper selected four cases of critical disease,including mistakenly sweating,hematemesis,measles pneumonia and cholelithiasis,and analyzed the theory and unique experience of using Aconiti Lateralis Radix Preparata to treat critical disease.[Results]MOU Yunfang was good at syndrome differentiation of Yin and Yang,cold and heat in the treatment of clinical critical diseases,flexible and ingenious use of Aconiti Lateralis Radix Preparata compatibility,and achieved good results.For example,the case of mistakenly sweating and Yang exhaustion was of Geyang syndrome with Yin-cold excess,and the treatment was based on the method of recuperating depleted Yang with Aconiti Lateralis Radix Preparata and Zingiberis Rhizoma;the case of hematemesis belonged to Qi desertion due to blood depletion and Yin deficiency affecting Yang,and the treatment was based on the method of warming and tonifying with Aconiti Lateralis Radix Preparata and Panax Ginseng;the case of measles pneumonia was of internal closure of pathogenic toxin and outward desertion of Yang Qi and the treatment was based on warming and heat-clearing method with Aconiti Lateralis Radix Preparata and Gypsum Fibrosum;the case of cholelithiasis was combined disease of Shaoyang and Yangming,with spleen deficiency and cold dampness,and the treatment was based on warming and purgating method with Aconiti Lateralis Radix Preparata and Rheum palmatum L.[Conclusion]MOU Yunfang’s experience in the treatment of critical disease with Aconiti Lateralis Radix Preparata compatibility is unique and effective,which is worthy of learning and reference.
