社区护士参与“医院-社区”联动模式的态度与行为调查分析

摘　　要：目的了解社区护士参与“医院-社区”联动模式的态度与行为。方法采用便利抽样法于2020年7—9月选取上海市96所社区医疗机构的264名社区护士作为研究对象。调查内容包括一般资料、社区护士对“医院-社区”联动项目的态度与行为。共发放问卷264份,有效问卷240份,问卷有效率为90.91%。结果态度得分方面,社区护士愿意参与“医院-社区”联动项目[(2.74±0.57)分]。社区护士认为“医院-社区”联动项目提升了其护理理论知识[(3.66±0.53)分]、康复护理技能[(2.80±0.63)分]、糖尿病及其相关并发症护理技能[(2.66±0.56)分]、社区急救技能[(2.57±0.55)分];认为“医院-社区”联动内容与实际护理内容不吻合[(2.38±0.70)分],在提升PICC技能[(1.96±0.46)分]、伤口或造口维护技能[(1.88±0.67)分]、各类风险评估技能[(1.73±0.59)分]等方面发挥的作用不显著;社区护士参与“医院-社区”联动项目的主动性偏低[(1.81±0.61)分];认为目前区域联动形式过于单一[(2.08±0.65)分]。行为得分方面,社区护士参与过的“医院-社区”联动项目内容中,50%以上社区护士参与的内容包括常见慢性病护理技术、康复运动指导、急救护理技术;参与形式主要是短期培训和专题讲座;61.67%(148/240)的社区护士开展频次为每半年1次;互动平台主要以微信交流群、短信或电话为主的社区护士分别占81.67%(196/240)、44.17%(106/240)。结论社区护士对“医院-社区”联动模式提升护理服务能力项目持较为积极态度,但是实际参与项目的积极主动性不够;还需进一步拓展“医院-社区”联动内容与形式,以全面提升社区护理人员的核心护理技能,切实满足社区广大患者的照护需求。Objective To understand the attitude and behavior of community nurses participating in the"hospital-community"linkage model.Methods Using the convenient sampling method,a total of 264 community nurses from 96 community medical institutions in Shanghai were selected as the research objects from July to September 2020.The survey content included general information and attitude and behavior of community nurses towards the"hospital-community"linkage project.A total of 264 questionnaires were distributed and 240 were valid questionnaires.The effective rate of the questionnaires was 90.91%.Results In terms of attitude scores,community nurses were willing to participate in the"hospital-community"linkage project[(2.74±0.57)].Community nurses believed that the"hospital-community"linkage project improved their theoretical nursing knowledge[(3.66±0.53)],rehabilitation nursing skills[(2.80±0.63)],diabetes and related complications nursing skills[(2.66±0.56)]and community first aid skills[(2.57±0.55)].The"hospital-community"linkage content of community nurses was not consistent with the actual nursing content[(2.38±0.70)].It did not play a significant role in improving PICC skills[(1.96±0.46)],wound or stoma maintenance skills[(1.88±0.67)]and various risk assessment skills[(1.73±0.59)].The initiative of community nurses to participate in the"hospitalcommunity"linkage project was low[(1.81±0.61)].The current regional linkage form of community nurses was too single[(2.08±0.65)].In terms of behavioral scores,in the"hospital-community"linkage project that community nurses participated in,more than 50%of community nurses participated in the content including common chronic disease nursing technology,rehabilitation exercise guidance,emergency nursing technology.The form of participation was mainly short-term training and special lectures.61.67%(148/240)of community nurses carried out once every six months.Community nurses whose interactive platforms were mainly based on WeChat exchange groups,text messages or phone call
