神经内镜辅助颅底显微神经外科手术治疗幕上胶质瘤的临床效果

摘　　要：目的:探讨神经内镜辅助显微神经外科手术治疗幕上胶质瘤的临床效果。方法:选取幕上胶质瘤患者102例,按照不同的治疗方法将其分为对照组和观察组,观察组52例,对照组50例。观察组采取神经内镜辅助显微神经外科手术治疗,对照组采取单纯颅底显微镜下肿瘤切除术治疗,比较两组患者肿瘤切除率、术后4周恢复情况、神经功能缺损(NIHSS)评分和日常生活活动能力(ADL)评分。结果:观察组患者全切率为92.31%,对照组患者全切率为78.00%,观察组的全切率高于对照组(P<0.05);观察组术后恢复良好率、中度残疾率均比对照组高,严重残疾、植物状态均比对照组低,差异具有统计学意义(P<0.05);治疗后,观察组患者的NIHSS评分比对照组低,ADL评分高于对照组,差异均有统计学意义(P<0.05)。结论:神经内镜辅助显微神经外科手术治疗幕上胶质瘤,可提高肿瘤切除率,降低复发率,改善患者病情,提高患者的日常生活活动能力。Objective:To investigate the clinical efficacy of neuroendoscopy-assisted microneurosurgery in the treatment of patients with supratentorial gliomas.Methods:102 patients with supratentorial gliomas were divided into the control group(50 cases)and observation group(52 cases)according to different treatment.The observation group were treated with neuroendoscopy-assisted microneurosurgery,while the control group were treated with simple skull base tumor resection under microscope.The tumor resection rate,recovery status,neurological deficit(NIHSS)score and activity of daily living(ADL)score at 4 weeks after surgery were compared between the two groups.Results:The total resection rate of patients was 92.31%in observation group,which was higher than 78.00%in control group(P<0.05);the rate of good postoperative recovery and moderate disability in the observation group was all higher than that in the control group,and the rate of severe handicapped and vegetative state was all lower than that in the control group,and the difference was statistically significant(P<0.05).After treatment,the NIHSS score in observation group was lower while the score of activity of daily living(ADL)was higher compared to that of control group(P<0.05).Conclusion:The treatment of neuroendoscopy-assisted microneurosurgery for patients with supratentorial gliomas can increase the resection rate,reduce the recurrence rate,improve the disease condition,and enhance the activity of daily living of patients.
