摘 要:目的探讨血浆可溶性podoplanin(s PDPN)在肺鳞癌诊断和预测中的临床价值。方法选取本院2018年9月-2020年9月的90例肺鳞癌患者作为病例组,并选取同期来院体检的100例健康者为对照组,应用小鼠抗人PDPN单克隆抗体SZ163和SZ168建立的酶联免疫吸附试验(ELISA)检测2组标本血浆中s PDPN的水平。绘制ROC曲线,并计算曲线下面积(AUC),评估s PDPN对肺鳞癌的早期诊断价值;采用Logistic回归模型分析s PDPN的预测价值。结果肺鳞癌组s PDPN水平(9.15±0.87) ng/ml高于健康对照组(2.16±0.17) ng/ml,P <0.000 1;s PDPN对诊断肺鳞癌的AUC为0.923。多因素Logistic回归分析显示s PDPN是肺鳞癌的独立预测因子(OR=4.827,95%CI:2.655~8.216,P <0.001)。结论血浆s PDPN可能成为一项新的肿瘤标志物,在肺鳞癌的早期诊断及预测中有一定的临床价值。Objective To investigate the clinical value of plasma soluble podoplanin(sPDPN)in the diagnosis and prediction of lung squamous cell carcinoma.Methods From September 2018 to September 2020,90 patients with lung squamous cell carcinoma admitted to our hospital were selected as case group and 100 health examinees in the same period were selected as the control group.The sPDPN levels in the plasma of the specimens were detected by an enzyme-linked immunosorbent assay(ELISA)established with mouse anti-human PDPN monoclonal antibodies SZ163 and SZ168.The ROC curve was drawn and the AUC area was calculated to evaluate the early diagnosis value of sPDPN for lung squamous cell carcinoma.Logistic regression model was used to analyze the predictive value of sPDPN.Results The sPDPN level in the lung squamous cell carcinoma group(9.15 ng/ml±0.87 ng/ml)was higher than that in the healthy control group(2.16 ng/ml±0.17 ng/ml),P<0.0001;the AUC area of sPDPN for the diagnosis of lung squamous cell carcinoma was 0.923.Multivariate Logistic regression analysis showed that sPDPN was an independent predictor of lung squamous cell carcinoma(OR=4.827,95%CI:2.655-8.216,P<0.001).Conclusion Plasma sPDPN may be used as a new tumor marker and has some certain clinical value in the early diagnosis and prediction of lung squamous cell carcinoma.
