摘　　要：目的探讨高分辨率磁共振成像(HR-MRI)在直肠癌术前TN分期及环周切缘(CRM)评估中的应用价值。方法选取经肠镜证实的63例直肠癌患者,术前均行HR-MRI检查,用SPSS24.0软件分析HR-MRI TN分期、CRM情况与病理TN分期、CRM情况的一致性。结果HR-MRI T分期与病理T分期的一致性良好(Kappa指数为0.822,P<0.01)。HR-MRI N分期与病理N分期的一致性一般(Kappa指数为0.600,P<0.01)。HR-MRI CRM累及情况与病理一致性良好(Kappa指数为0.893,P<0.01)。结论HR-MRI能较好地显示肿瘤的浸润深度、淋巴结转移及CRM的侵犯情况,对直肠癌术前TN分期和CRM的评估有一定的临床应用价值。Objective To explore the application value of high-resolution MRI(HR-MRI)in assessment of TN staging and circumferential resection margin(CRM)of preoperative rectal cancer.Methods Sixty-three patients with rectal cancer confirmed by colonoscopy were selected.All of them were underwent HR-MRI before operation.A SPSS 24.0 software was used to analyze the consistencies of TN staging and CRM between HR-MRI and pathology.Results The consistencies between HR-MRI and pathology in assessment of the T staging and the CRM involvement were good(Kappa indexes were 0.822 and 0.893,both P<0.01),meanwhile the consistency of the N staging was indifferent(Kap pa index was 0.600,P<0.01).Conclusion HR-MRI can preferably show the infiltration depth of tumor,lymph node metastasis and invasion of CRM,and it has certain clinical application value in assessing the TN staging and CRM of preoperative rectal cancer.
