摘 要:目的 分析血清基质金属蛋白酶-7(MMP-7)、基质金属蛋白酶-9(MMP-9)及基质金属蛋白酶组织抑制因子-1(TIMP-1)表达与婴幼儿血管瘤瘤体增生的关系。方法 回顾分析2018年1月-2019年10月医院完成治疗的33例增生期血管瘤患儿资料,作为观察组;并选取同期医院完成治疗的33例消退期血管瘤患儿资料,作为对照组。统计并对比2组患儿一般资料、各血清指标,重点分析血清MMP-7、MMP-9及TIMP-1表达与婴幼儿血管瘤瘤体增生的关系。结果 观察组血清碱性成纤维细胞生长因子(bFGF)、血管内皮生长因子(VEGF)、MMP-7、MMP-9水平均高于对照组,且TIMP-1水平低于对照组(P<0. 05);组间其他资料比较差异无统计学意义(P> 0. 05)。经回归分析结果显示,血清bFGF、VEGF、MMP-7、MMP-9过表达、TIMP-1低表达均是婴幼儿血管瘤瘤体增生的风险因子(P<0. 05)。绘制ROC曲线,结果显示,血管瘤患儿血清MMP-7、MMP-9、TIMP-1水平预测婴幼儿血管瘤瘤体增生的AUC> 0. 80,预测价值均较理想。相关性分析结果显示,血管瘤患儿血清MMP-7、MMP-9水平之间呈正相关(r=0. 451,P<0. 05),血清MMP-7、MMP-9水平均与TIMP-1水平之间呈负相关(r值分别为-0. 262、-0. 338,P<0. 05)。结论 血清MMP-7、MMP-9过表达、TIMP-1低表达与血管瘤患儿瘤体增生相有关,可能是血管瘤患儿瘤体增生风险因子,并可指导患儿早期瘤体增生风险的预测与评估。Objective To analyze the relationship between the expression of serum matrix metalloproteinase-7(MMP-7),metalloproteinase-9(MMP-9)and tissue inhibitor of metalloproteinase-1(TIMP-1)and the proliferation of infantile hemangioma.Methods The clinical data of 33 children with proliferative hemangioma who completed treatment in the hospital from January 2018 to October 2019 were retrospectively analyzed and classified as the observation group;the data of 33 patients with hemangioma in the regression period who completed treatment in the same period were selected and classified as the control group.The general data and each serum index of children in the two groups were compared,and the relationship between the expression of serum MMP-7,MMP-9 and TIMP-1 and the proliferation of infantile hemangioma was mainly analyzed.Results The levels of basic fibroblast growth factor(bFGF),vascular endothelial growth factor(VEGF),MMP-7 and MMP-9 in the observation group were higher than those in the control group,and the level of TIMP-1 was lower than that in the control group(P<0.05);compared other data between groups,the difference was not statistically significant(P>0.05);the results of regression analysis showed that overexpression of serum bFGF,VEGF,MMP-7,MMP-9 and low expression of TIMP-1 were the risk factors of infantile hemangioma proliferation(P<0.05).ROC curve was drawn,and the results showed that serum MMP-7,MMP-9 and TIMP-1 levels in children,with hemangioma predicted the AUC of proliferation of infantile hemangioma was>0.80,and the predictive value was ideal;correlation analysis showed that there was a positive correlation between serum MMP-7 and MMP-9 levels(r=0.451<0.05),and the serum levels of MMP-7 and MMP-9 were negatively correlated with TIMP-1(r=-0.262,r-0.338,P<0-05).Conclusion The overexpression of MMP-7-and MMP-9 and the low expression of TIMP-1 are associated with the proliferation of hemangioma in children,and may be a risk factor of tumor proliferation in children with hemangioma,and can guide the predic
