摘　　要：目的:探讨胰腺癌组织中Klotho蛋白的表达水平对患者临床病理特征和5年生存率的影响。方法:收集2010年1月—2015年1月在南阳市第二人民医院行胰腺癌手术患者的胰腺癌组织和对应的癌旁组织;免疫组织化学检测组织中Klotho蛋白表达情况,根据Klotho蛋白表达将患者分为阳性表达组和阴性表达组;采用χ^(2)检验比较Klotho蛋白阳性表达与阴性表达组间的临床特征,绘制不同组间Kaplan-Meier生存曲线并行Log-rank检验;Cox单因素和多因素回归分析影响胰腺癌预后的危险因素。结果:胰腺癌组织中Klotho蛋白表达阳性率为63.0%,显著低于癌旁组织的98.0%(χ^(2)=39.019,P<0.001);Klotho阳性表达与阴性表达组间年龄、淋巴结转移、TNM分期、分化程度、周围神经浸润之间差异有统计学意义(P<0.05);Klotho阳性表达组5年生存率为14.3%,高于Klotho阴性表达组的5.4%(χ^(2)=4.698,P=0.030)。淋巴结转移、TNM分期、分化程度、周期神经浸润、Klotho表达均为影响胰腺癌预后的独立危险因素(P<0.05)。结论:Klotho在胰腺癌组织中阳性表达率降低,与患者年龄、淋巴结转移、TNM分期、分化程度、周围神经浸润相关,并降低患者5年生存率。Objective:To detect the effect of Klotho protein expression in pancreatic cancer on clinical characteristics and 5-year survival rate.Methods:Collect pancreatic cancer tissues and corresponding adjacent tissues of patients undergoing pancreatic cancer surgery in Nanyang Second People's Hospital from January 2010 to January 2015;Immunohistochemical detection of Klotho protein expression in the tissues,and patients were divided into positive expression group and negative expression group according to Klotho protein expression;Chi-square test was used to compare the clinical features of the Klotho protein positive and negative expression groups,and Kaplan-Meier survival curves between different groups were plotted in parallel Log-rank test;Cox univariate and multivariate regression analysis affect the risk factors of pancreatic cancer prognosis.Results:The positive rate of Klotho protein expression in pancreatic cancer was 63.0%,which was significantly lower than that of pancreatic cancer adjacent tissues,98.0%(χ^(2)=39.019,P<0.001);There were statistical differences between Klotho positive and negative expression groups in age,lymph node metastasis,TNM stage,degree of differentiation,and peripheral nerve infiltration(P<0.05);The 5-year survival rate of patients in the Klotho positive expression group was 14.3%,which was higher than the 5.4%survival rate of patients in the Klotho negative expression group.There was a statistical difference between the groups(χ^(2)=4.698,P=0.030);Lymph node metastasis,TNM stage,degree of differentiation,periodic nerve infiltration and Klotho were all independent risk factors that affect the prognosis of pancreatic cancer(P<0.05).Conclusion:The positive expression rate of Klotho in pancreatic cancer is reduced,which is related to the patient's age,lymph node metastasis,TNM stage,degree of differentiation,peripheral nerve infiltration,and reduces the patient's 5-year survival rate.
