摘　　要：目的探讨颏下岛状瓣临床运用中彩色多普勒超声(CDUS)的作用和意义。方法选取2013年7月至2018年6月江油市第二人民医院收治的口腔鳞癌患者40例,按照随机数字表法分为研究组和对照组,每组20例。研究组通过CDUS检查了解面动脉情况和转移淋巴结情况后进行手术治疗,对照组经常规检查后进行手术治疗。比较2组患者临床疗效。结果2组患者术前转移淋巴结检出率比较,差异无统计学意义(P>0.05);术后病理检查诊断转移淋巴结比较,差异有统计学意义(P<0.05)。研究组患者术后复发率明显低于对照组,差异有统计学意义(P<0.05);2组患者术后皮瓣存活率比较,差异无统计学意义(P>0.05)。结论CDUS检查对颏下岛状瓣临床运用可降低术后复发风险,适合临床推广应用。Objective To explore the role and significance of color Doppler ultrasound(CDUS)in the clinical application of submental island flap.Methods A total of 40 patients with oral squamous cell carcinoma admitted to the Second People′s Hospital of Jiangyou City from July 2013 to June 2018 were selected.The patients were randomly divided into two groups using the means of random number table,which included the study group and the control group.There were 20 cases in each group.In the study group,the location of patients′facial artery and metastatic lymph nodes were examined by the CDUS before the surgery,while in the control group the procedures were conducted under the routine examination before the operation.The clinical effects of the two groups were compared.Results There was no significant difference in the detection rate of the metastatic lymph nodes between the two groups(P>0.05).There was significant difference in the postoperative pathological diagnosis of metastatic lymph nodes between the two groups(P<0.05).The risk of cancer recurrence in the study group was significantly lower than that in the control group,and the difference was statistically significant(P<0.05).There was no difference in the postoperative flap survival rate between the two groups(P>0.05).Conclusion The CDUS in the clinical application of submental island flap can reduce the risk of postoperative recurrence,which is suitable for clinical application.
