慢性根尖周炎患者首次根管治疗失败的影响因素

摘　　要：目的探讨慢性根尖周炎患者首次根管治疗失败的影响因素,为临床成功治疗该病提供参考。方法回顾性分析2017年7月至2020年8月本院收治的80例慢性根尖周炎患者的临床资料,统计患者性别、病程、根管治疗手术时间等临床资料及首次根管治疗失败情况,分析慢性根尖周炎患者首次根管治疗失败的影响因素。结果80例慢性根尖周炎患者中首次根管治疗失败18例,失败率为22.5%。经单因素及多因素分析结果显示,髓腔预备不当、根管预备不当、无菌操作不当、根管充填不当、冠渗漏或冠方修复体不良、器械折断、并发牙周牙髓联合病变、继发龋均为慢性根尖周炎患者首次根管治疗失败的危险因素(OR>1,P<0.05)。结论髓腔预备不当、根管预备不当、无菌操作不当、根管充填不当、冠渗漏或冠方修复体不良、器械折断、并发牙周牙髓联合病变、继发龋会导致慢性根尖周炎患者首次根管治疗失败,临床可据此实施相关预防措施以降低根管治疗失败率。Objective To explore the influencing factors of first root canal treatment failure in patients with chronic periapical periodontitis,and to provide reference for clinical successful treatment of the disease.Methods The clinical data of 80 patients with chronic periapical periodontitis treated in our hospital from July 2017 to August 2020 were analyzed retrospectively.The clinical data such as gender,course of disease,operation time of root canal therapy and the failure of the first root canal therapy were counted,and the influencing factors of the failure of the first root canal therapy in patients with chronic periapical periodontitis were analyzed.Results Among 80 patients with chronic periapical periodontitis,the first root canal treatment failed in 18 cases,and the failure rate was 22.5%.The results of univariate and multivariate analysis showed that improper medullary cavity preparation,improper root canal preparation,improper aseptic operation,improper root canal filling,crown leakage or poor crown restoration,instrument fracture,combined periodontal pulp lesions and secondary caries were the risk factors for the failure of the first root canal treatment in patients with chronic periapical periodontitis(OR>1,P<0.05).Conclusion Improper medullary cavity preparation,improper root canal preparation,improper aseptic operation,improper root canal filling,crown leakage or poor crown restoration,instrument fracture,combined periodontal pulp lesions and secondary caries will lead to the failure of first root canal treatment in patients with chronic apical periodontitis.Relevant preventive measures can be implemented to reduce the failure rate of root canal treatment.
