摘  要:目的:探讨全自动DNA图像分析技术(DNA-ICM)在胸水细胞病理学检查中的临床应用价值。方法:选择558例病例,分别采用薄层液基细胞学联合细胞蜡块技术(TCT+CB)和Feulgen染色的DNA-ICM技术进行检测,并将两种结果进行分析。结果:558例中,TCT+CB检出阳性173例,阴性385例,阳性检出率为31.00%(173/558),阴性检出率为69.00%(385/558);DNA-ICM检出阳性253例,阴性305例,阳性检出率为45.34%(253/558),阴性检出率为54.66%(305/558);DNA-ICM在恶性肿瘤诊断中的敏感性、特异性分别为95.95%(166/173)、77.40%(298/385)。DNA-ICM阳性且TCT+CB诊断为阴性的45例病例中,经临床综合诊断为阳性的32例。结论:细胞DNA-ICM的敏感性和特异性均较高,而且可有效提高胸水细胞学的阳性检出率,有效减少细胞病理学诊断的漏检。Objective:To explore the clinical value of full-automatic DNA image cytometry(DNA-ICM)in pleural effusion cancer screening.Methods:A total of 558 cases were selected and detected by TCT+CB and Feulgen staining,respectively.The two results were analyzed.Results:Among the 558 cases,173 cases were positive detected by"TCT+CB",with a malignant detection rate of 31.00%(173/558),and 385 cases were negative,with a positive detection rate of 69.00%(385/558).Full-automatic DNA image analysis was found in 253 positive cases,with a positive detection rate of 45.34%(253/558),and 305 negative cases,with a negative detection rate of 54.66%(305/558).The sensitivity and specificity of cellular DNA-ICM in the diagnosis of malignant tumors were 95.95%(166/173)and 77.40%(298/385),respectively.Among the 45 cases diagnosed as positive for DNA-ICM but negative for TCT+CB,32 cases were diagnosed as positive by clinical comprehensive diagnosis.Conclusion:The sensitivity and specificity of cell DNA-ICM are high,and the positive detection rate of pleural fluid cytology can be effectively increased,and the missed detection of cell pathology diagnosis can be effectively reduced.
