临床护士死亡态度对其临终关怀态度影响的研究

摘　　要：目的了解临床护士临终关怀态度和死亡态度的现状,探究护士临终关怀态度的影响因素。方法采用一般资料问卷、临终关怀态度量表及死亡态度描绘量表对803名临床护士进行问卷调查。结果临床护士临终关怀态度总分为(97.96±8.43)分,死亡态度最趋向于自然接受,条目均分为(3.92±0.52)分,临床护士临终关怀态度与自然接受的死亡态度呈正相关(r=0.367,P<0.01),与死亡恐惧、死亡逃避、趋近接受和逃离接受4个维度呈负相关(r=-0.105~-0.348,P<0.01)。多元线性回归分析显示,年龄、职称、工作科室、关于死亡的过往教育情况、以往照护临终患者的经历、自然接受、死亡逃避、逃离接受是临床护士临终关怀态度的主要影响因素(P<0.01或P<0.05)。结论临床护士临终关怀态度积极性不高,与其缺乏正确的生死观有关,合理的教育内容和手段以及良好的支持环境将有望促进临床护士临终关怀态度的正向发展。Objective To investigate the status quo of clinical nurses'attitudes towards death and hospice care,and explore the influencing factors of nurses'attitudes towards hospice care.Methods Totally,803 nurses were investigated with general information questionnaire,Frommelt Attitudes Toward Care of the Dying Scale Form-B(FATCOD-B),and Death Attitude Profile-Revised(DAP-R).Results The total score of FATCOD-B was(97.96±8.43).The attitude towards death tended to be neutral acceptance,and the average score of each item was(3.92±0.52).The score of FATCOD-B was positively correlated with the neutral acceptance dimension(r=0.367,P<0.01),and negatively correlated with death fear,death escape,approach acceptance and escape acceptance(r=-0.105~-0.348,P<0.01).Multiple linear regression analysis showed that age,title,department of work,previous education about death,previous experience of caring for terminal patients,natural acceptance,death avoidance and escape acceptance were the main influencing factors of hospice care attitude of clinical nurses(P<0.01 or P<0.05).Conclusion Clinical nurses have less positive attitudes towards hospice care,which was related to their lack of a correct view of life and death.Reasonable educational content and methods and a good supporting environment are expected to promote the positive development of their attitudes of caring for dying patients.
