整合医学观念补充及其对新型冠状病毒肺炎疫情防控的探讨

摘　　要：有鉴于欧美一些国家“非真正封城”的措施不奏效,而上海没封城也没搞全员检测却迅速控制住2021年1月新型冠状病毒肺炎(COVID-19)疫情,笔者在对COVID-19疫情及其防控的复杂性特点与规律进行探讨与分析的基础上,认为整合医学(HIM)是新时期医药学发展的必由之路,将其核心“整体观、整合观和医学观”补充修正为“整体观”“整合观”“复杂性科学观”与“病证结合法”(三观一法),不仅更能体现其认识特点,而且也使其在实践中能够“有理可循、有法可依”。由于COVID-19多因素致病的复杂性多变特点,不仅使其认识与诊断要多种方法并举,如将新型冠状病毒(SARS-CoV-2)核酸检测、抗体检测与临床特征等相结合,而且其防控与防治也要根据不同的情况,整合不同的措施与方法。而这只有从HIM“三观一法”的角度来认识与把握,也许才能更加全面与准确地掌握COVID-19发生、防控与防治的精髓与规律,进而更加科学精准地应用“封闭(封城)”、疫苗及各种防治药物与方法,从而达到最佳的防控与防治效果。Considering that the“non-real city closure”measures in some countries like Europe and the United States did not work,and that Shanghai has quickly control COVID-19 pandemic situation without“city closure”or the full-staff PCR test in January 2021,based on the discussion and analysis of the complexity characteristics and rules of COVID-19 pandemic and its prevention and control,we hold that holistic integrative medicine(HIM)is the only development way of medicine in the new period,and revise its core ideas“holism view”,“integration view”and“medical view”into“holism view”,“integration view”,“complexity science view”and“combination of Zheng condition differentiation and disease differentiation”(three views and one method),which can not only better embody its cognitive characteristics,but also make it more reasonable and logical.Owing to the complex and changeable characteristics of multiple pathogenic factors of COVID-19,not only does it require a combination of multiple methods for its understanding and diagnosis,such as SARS-CoV-2 nucleic acid detection,antibody detection and clinical manifestations,but also need to integrate different measures and methods according to different conditions for its control.From the perspective of“three views and one method”,we can understand and grasp the essence and law of COVID-19 occurrence,prevention and control more comprehensively and accurately,and then apply“closure(city closure)”,vaccines and various prevention and control drugs and methods more scientifically and accurately,so as to achieve the best prevention and control effect.
