摘  要:目的 探索分析针对鼻咽癌患者应用EB病毒核抗原抗体(NA-IgA)、ZTA蛋白IgA抗体(ZTA-IgA)以及衣壳抗原抗体(VCA-IgA)单独检测及联合检测方式实施筛查的临床价值。方法 选择我院2015年1月至2019年12月收治的鼻咽癌患者40例作为研究组,并选择同期健康体检者40例作为对照组,分别接受酶联免疫吸附测定法血清EB病毒NA-IgA、ZTA-IgA和VCA-IgA单独检测及联合检测,将效果进行对比。结果 三项单项检测中,ZTA-IgA灵敏度、特异度为最高;NA-IgA灵敏度、特异度为次之;VCA-IgA特异度为最低。三项联合检测的灵敏度、特异度为最高;显著性超过NA-IgA联合VCA-IgA灵敏度的94.59%以及特异度的88.37%,NA-IgA联合ZTA-IgA灵敏度的94.74%以及特异度的90.48%,ZTA-IgA联合VCA-IgA灵敏度的94.59%以及特异度的88.37%。结论 针对鼻咽癌积极实施EB病毒NA-IgA、ZTA-IgA和VCA-IgA三项抗体联合检测,有助于提升诊断灵敏度及特异度,对于提升鼻咽癌筛查准确性具有重要价值。Objective To explore and analyze clinical screening value of EB virus nuclear antigen antibody (Na IGA),ZTA protein IgA antibody (ZTA IGA) and coat antigen antibody (VCA IGA) alone and jointly for NPC patients.Methods The paper chose 40 cases NPC patients treated in our hospital from January 2015 to December 2019 as study group,and 40 cases healthy physical examination subjects at the same period as control group.Serum EB virus NA IgA,ZTA IgA and VCA IgA were detected separately and jointly by enzyme-linked immunosorbent assay,results were compared between groups.Results ZTA IgA had the highest sensitivity and specificity among three single tests;Na IgA has the second highest sensitivity and specificity;VCA IgA was the lowest.Combination tests had the highest sensitivity and specificity,higher than sensitivity 94.59% and specificity 88.37% of Na IgA combined with VCA IgA,sensitivity 94.74%,specificity 90.48% of Na IgA combined with ZTA IgA,sensitivity 94.59% and specificity 88.37% of ZTA-IgA combined with VCA-IgA.Conclusion Combination of EB virus NA IgA,ZTA IgA and VCA IgA in NPC is helpful to improve sensitivity and specificity of diagnosis,which has great value to improve accuracy of NPC screening.
