保乳手术和改良根治术治疗早期乳腺癌的效果比较

摘　　要：目的比较保乳手术和改良根治术治疗早期乳腺癌的临床效果。方法回顾性分析2018年11月至2019年12月郑州人民医院收治的178例早期乳腺癌患者的临床资料,将采用改良根治术的124例患者作为对照组,采用保乳手术的54例患者作为研究组。比较两组围术期情况(手术时间、切口长度、术中引流量、术中出血量、住院时间)、并发症发生率、乳房外形优良率、预后情况(生存率、复发率、转移率)。结果研究组切口长度、手术时间、住院时间短于对照组,术中引流量、术中出血量少于对照组,差异有统计学意义(P<0.05)。研究组并发症发生率(3.85%,2/54)低于对照组(13.71%,17/124),差异有统计学意义(P<0.05)。研究组乳房外形优良率(94.44%,51/54)高于对照组(81.45%,101/124),差异有统计学意义(P<0.05)。研究组生存率、复发率、转移率与对照组比较差异未见统计学意义(P>0.05)。结论与改良根治术相比,保乳手术治疗早期乳腺癌患者,手术切口小,手术时间短,可减少术中引流量和出血量,降低并发症发生率,加快患者康复进程,提高乳房外形优良率,满足患者美观需求,且预后良好。Objective To compare the application value of breast-conserving surgery versus modified radical mastectomy in the treatment of early breast cancer.Methods The clinical data from 178 patients with early breast cancer treated in Zhengzhou People’s Hospital from November 2018 to December 2019 were retrospectively analyzed.Among them,124 patients who underwent modified radical mastectomy were selected as the control group,and 54 patients who underwent breast-conserving surgery were selected as the study group.The perioperative conditions(operation time,incision length,intraoperative drainage volume,intraoperative blood loss,hospital stay),complication rate,excellent and good rate of breast appearance,and prognosis(survival rate,recurrence rate,metastasis rate)were compared between the two groups.Results The length of incision,operation time,and hospital stay of the study group were shorter than those in the control group,and the intraoperative drainage volume and intraoperative blood loss of the study group were less than those of the control group(P<0.05).The complication rate of the study group was 3.85%(2/54),lower than the 13.71%(17/124)of the control group(P<0.05).The excellent and good rate of breast appearance of the study group was 94.44%(51/54),which was higher than the 81.45%(101/124)of the control group(P<0.05).There was no significant difference in survival rate,recurrence rate and metastasis rate between the study group and the control group(P>0.05).Conclusions Compared with modified radical mastectomy,breast conserving surgery,has small incision and short operation time in the treatment of patients with early breast cancer,and it can reduce the amount of drainage and bleeding,reduce the incidence of complications,accelerate the rehabilitation process of patients,improve the excellent and good rate of breast appearance,which can meet the aesthetic needs of patients,with good prognosis.
