摘 要:目的调查不同强度有氧训练对卒中后恢复期患者躯体运动功能、耐力、抑郁的影响。方法抽取郑州大学第二附属医院2018年1月至2019年12月收治的脑卒中患者53例进行随机对照试验,采用分层随机分组法将纳入患者分为研究组(27例)与对照组(26例)。研究组进行60%~80%心率储备的高强度有氧跑步机训练,对照组进行40%以下心率储备的室外步行训练。两组均训练16周。比较两组低能耗活动时间、抑郁[汉密顿抑郁量表(HAMD)评分]、躯体运动功能[Fugl-Meyer运动量表评分、日常生活能力量表(ADL)评分]及耐力情况[6 min步行测试(6MWT)评分、递增式往来步道测试(ISWT)评分]。结果训练后,两组HAMD评分、6MWT评分、ISWT评分、ADL评分、Fugl-Meyer评分均优于训练前(P均<0.05),且研究组6MWT评分、ISWT评分、Fugl-Meyer评分、ADL评分优于对照组(P<0.05)。但两组训练后体力活动水平、低能耗活动时间与训练前比较差异未见统计学意义(P>0.05)。结论仅靠有氧运动并不能有效提高体力活动水平,但可改善卒中后恢复期患者的抑郁、躯体活动能力和耐力,且较高强度的有氧运动效果更加明显。Objective To investigate the effects of aerobic training with different intensities on motor abilities,endurance,depression of stroke patients during recovery.Methods A total of 53 patients with stroke treated in the Second Affiliated Hospital of Zhengzhou University from January 2018 to December 2019 were enrolled in the randomized controlled trial.And they were divided into study group(27 cases)and control group(26 cases)by stratified random grouping method.Patients in study group had aerobic training by running machine,with 60%-80%heart rate reserve.Patients in control group had outside walk,with≤40%heart rate reserve.Patients in both groups exercised for 16 weeks.The time of low energy consumption activities,depression assessed by Hamilton Depression scale(HAMD),motor abilities assessed by Fugl-Meyer motor scale,activity of daily life(ADL),and endurance assessed by six-minute walk test(6MWT)and incremental shuttle-walk test(ISWT)were compared between the two groups.Results After training,the HAMD score,6MWT score,ISWT score,ADL score and Fugl-Meyer score of the two groups were higher than those before training(P<0.05).And the 6MWT score,ISWT score,ADL score and low-extremity Fugl-Meyer score of study group were superior to those of control group after training(P<0.05).There was no significant difference in physical activity level,low energy consumption activity time before and after training in neither group(P>0.05).Conclusions Aerobic exercise alone can not effectively improve the level of physical activity,but it has effects on improving depression,motor ability,endurance in stroke patients during recovery.And high-intensity aerobic training has a more significant effect.
