盐酸米诺环素联合多联抗生素糊剂治疗牙周牙髓联合病变的效果观察

摘　　要：目的分析盐酸米诺环素联合多联抗生素糊剂治疗牙周牙髓联合病变的临床效果。方法抽取2019年2月至2021年2月漯河市中医院收治的牙周牙髓联合病变患者164例,按随机数字表法分为观察组与对照组,每组82例。对照组采用根管冲洗常规治疗,观察组在对照组基础上采用盐酸米诺环素联合多联抗生素糊剂治疗,对比两组疗效和治疗前后牙齿松动度、牙周微生态环境[牙周附着水平(ALI)、菌斑指数(PLI)、牙龈指数(GI)]、炎性状态[白细胞介素-17(IL-17)、C反应蛋白(CRP)、白细胞介素-1β(IL-1β)]。结果观察组总有效率为95.12%(78/82),高于对照组的84.15%(69/82),P<0.05。治疗后,两组牙齿松动度均较治疗前降低,且观察组低于对照组(P<0.05)。治疗后,两组ALI、PLI、GI均较治疗前降低,且观察组低于对照组(P<0.05)。治疗后,两组血清IL-17、CRP、IL-1β水平均低于治疗前,且观察组低于对照组(P<0.05)。结论盐酸米诺环素联合多联抗生素糊剂治疗牙周牙髓联合病变的效果显著,可降低牙齿松动度并缓解炎性状态,改善牙周微生态环境。Objective To analyze the effects of minocycline hydrochloride combined with multicombination antibiotic paste on patients with periodontic endodontic lesion.Methods A total of 164 patients with periodontic endodontic lesion treated in Luohe Hospital of Traditional Chinese Medicine from February 2019 to February 2021 were selected,and they were divided into observation group and control group according to random number table method,with 82 cases in each group.The control group was treated by routine root canal irrigation,and the observation group was treated with minocycline hydrochloride combined with multi-combination antibiotic pasture on the basis of the control group.The curative effect were compared between the two groups,and the degree of tooth looseness,periodontal microenvironment,including periodontal attachment levels(ALI),plaque index(PLI),gingival index(GI),inflammatory status assessed by interleukin-17(IL-17),C-reactive protein(CRP)and interleukin 1β(IL-1β)were compared between the two groups before and after treatment.Results The total effective rate of observation group was 95.12%(78/82),higher than the 84.15%(69/82)of control group(P<0.05).After treatment,the degree of tooth looseness in the two groups was lower than that before treatment,and that in observation group was lower than that in control group(P<0.05).After treatment,ALI,PLI and GI in the two groups were decreased,compared with before treatment,and these indexes in observation group were lower than those in control group(P<0.05).After treatment,the levels of serum IL-17,CRP and IL-1βin the two groups were lower than those before treatment,and those in observation group were lower than those in control group(P<0.05).Conclusions Minocycline hydrochloride combined with multicombination antibiotic paste has a significant effect on patients with periodontic endodontic lesion,which can reduce the degree of tooth looseness,relieve the inflammatory status,and improve the periodontal microenvironment.
