摘  要:目的探究多发性骨髓瘤患者合并感染的危险因素及相关控制措施。方法选择多发性骨髓瘤患者125例,根据患者是否发生感染,分为感染组59例,未感染组66例。对引发感染的单因素进行分析,并对多发性骨髓瘤合并感染的相关危险因素行多因素分析。结果感染组化疗周期>8个、体力活动状态评分>1分、白蛋白水平20~30 g/L、β2微球蛋白≥5.5 mg/L、血红蛋白60~100 g/L、中性粒细胞数≤1.5×10^(9)/L发生率均高于未感染组,差异有统计学意义(P<0.05);Logistic回归分析可知,化疗周期>8个、体力活动状态评分>1分、白蛋白水平≤30 g/L、β2微球蛋白≥5.5 mg/l、血红蛋白60~100 g/L、中性粒细胞数≤1.5×10^(9)/L是多发性骨髓瘤患者并发感染的独立危险因素(OR>1,P<0.05)。结论加强日常护理、预防性使用抗菌或抗病毒药物、加强营养支持等有助于降低多发性骨髓瘤患者感染几率。Objective To investigate the risk factors and control measures of multiple myeloma complicated with infection.Methods 125 patients with multiple myeloma were divided into 59 cases of infected group,and 66 cases of uninfected group.The single factor that caused infection,and risk factors of multiple myeloma complicated with infection were analyzed.Results The infected group,the incidence of chemotherapy cycle>8,physical activity status score>1,albumin level 20~30 g/L,β2 microglobulin≥5.5 mg/l,hemoglobin 60~100,neutrophil number<1.5×10^(9)/L were higher than those of uninfected group,the difference was statistically significant(P<0.05);Logistic regression analysis showed,chemotherapy cycle>8,physical activity status score>1,albumin level≤30 g/L、β2 microglobulin≥5.5,hemoglobin 60~100 g/L、neutrophil number 1.5×10^(9)/were independent risk factors for multiple myeloma patients complicated with infection(OR>1,P<0.05).Conclusion Strengthen daily care,preventive use of antimicrobial or antiviral drugs,and strengthen nutritional support helps to reduce the risk of infection of multiple myeloma complicated with infection.
