结膜入路中采用指触钝性分离对治疗眼眶血管瘤的疗效及预后的影响

摘　　要：目的探讨结膜入路中采用指触钝性分离对治疗眼眶血管瘤的疗效及预后的影响。方法选取39例眼眶深部海绵状血管瘤患者,根据治疗方式的差异分为2组。对照组20例采取结膜切口分离入眶治疗,而研究组19例在对照组基础上联合指触钝性分离,比较2组临床疗效等差异。结果研究组的手术时间短于对照组(P<0.05),而2组术后肿瘤残留率及视力恢复程度比较差异不显著(P>0.05);术后研究组总并发症发生率较对照组低(P<0.05)。研究组术后患者的生活质量较对照组高(P<0.05)。结论结膜入路中采用指触钝性分离可有效加快眼眶血管瘤手术进程,同时不损伤周围组织并降低术后并发症发生率,且能提高患者的日常生活质量,值得临床推广应用。Objective To investigate the curative effect and prognosis of orbital hemangioma patients with blunt finger contact separation through conjunctival approach.Methods 39 cases of deep orbital cavernous hemangioma were selected.They were divided into 2 groups according to the difference of treatment methods.The control group(20 cases)was treated with conjunctival incision separation into orbit,while the study group(19 cases)was treated with finger contact blunt separation on the basis of the control group.Results The operation time of the study group was shorter than the control group(P<0.05),and there was no difference in the postoperative tumor residual rate and the degree of visual acuity recovery between the 2 groups(P>0.05).The total incidence of complications of the study group was lower than the control group(P<0.05);The quality of life of the study group was higher than the control group(P<0.05).Conclusion In conjunctival approach,finger contact blunt separation can effectively speed up the operation process of orbital hemangioma patients,at the same time,it does not damage the surrounding tissue and reduce the incidence of postoperative complications,and it can improve the quality of daily life of patients,which is worthy of clinical application.
