摘  要:目的研究血清DCP、NK细胞和CD4^(+)、CD8^(+)水平与原发性肝癌(PHC)患者病情严重程度的关系。方法选取125例原发性肝癌患者作为研究对象,另选取乙肝潜伏期患者、肝硬化患者以及表观健康者各125例作为对照,比较4组的DCP、NK细胞和CD4^(+)、CD8^(+)水平之间的差异,分析PHC不同病理患者的DCP、NK细胞和CD4^(+)、CD8^(+)水平之间的差异,研究PHC不同病理状态与DCP、NK细胞和CD4^(+)、CD8^(+)水平的相关性。结果4组研究对象的DCP、NK细胞和CD4^(+)、CD8^(+)水平之间的差异存在统计学意义(P<0.05),通过两两比较,4组研究对象的DCP、CD8^(+)表达水平从高到低依次为原发性肝癌组、肝硬化组、乙肝组、健康组,4组研究对象的NK细胞、CD4^(+)表达水平从低到高依次为原发性肝癌组、肝硬化组、乙肝组、健康组(P<0.05)。随着PHC患者的淋巴结以及肝内转移,患者的DCP、CD8^(+)表达水平越高,NK细胞、CD4^(+)水平越低(P<0.05);原发性肝癌患者的组织分化、淋巴结转移、TNM分期、肝内转移情况与患者的DCP、CD8^(+)表达水平呈现正相关,与患者的NK细胞、CD4^(+)水平呈现负相关。通过ROC曲线分析,早期肝癌的DCP、NK细胞和CD4^(+)、CD8^(+)水平临界值分别为2049.11 mAU/mL,32.67%,24.19%,16.71%。结论血清DCP、NK细胞和CD4^(+)、CD8^(+)水平与PHC发生发展显著相关,可作为疾病进展以及治疗效果的重要评价依据。Objective To study the relationship between the level of serum DCP,NK cells,CD4^(+),CD8^(+) and the severity of PHC.Methods 125 patients with primary liver cancer diagnosed were selected as the study object,and 125 patients with hepatitis B incubation period,cirrhosis and apparent healthy patients were selected as the control.The differences among DCP,NK cells,CD4^(+),CD8^(+) levels in the 4 groups were compared,and the differences among DCP,NK cells,CD4^(+),CD8^(+) levels in different pathological patients were analyzed.The correlation between pathological state and DCP,NK cells,CD4^(+),CD8^(+) levels.Results The DCP,NK cells,CD4^(+),CD8^(+) levels of the four groups were statistically significant(P<0.05).The DCP and CD8^(+) expression levels of the 4 groups were from high to low in primary liver cancer group,cirrhosis group,hepatitis B group and health group,and the NK cells and CD4^(+) expression levels of the 4 groups were from low to high in primary liver cancer group,cirrhosis group,hepatitis B group and health group(P<0.05).With the lymph node and intrahepatic metastasis,the higher the expression level of DCP and CD8^(+),the lower the level of NK cells and CD4^(+)(P<0.05).Tissue differentiation,lymph node metastasis,TNM stage and intrahepatic metastasis were positively correlated with DCP and CD8^(+) expression,and negatively correlated with NK cells and CD4^(+) expression.According to ROC curve analysis,the critical values of DCP,NK cells,CD4^(+),CD8^(+) were 2049.11 Mau/ml,32.67%,24.19%,16.71%respectively.Conclusion The serum levels of DCP,NK cells,CD4^(+),CD8^(+) are significantly related to the development of PHC patients,which can be used as an important basis for future disease progress and treatment effect.
