摘　　要：目的探讨西门子双源CT(DSCT)对胰腺实性假乳头状瘤(SPTP)与神经内分泌肿瘤(PNET)的鉴别价值。方法选取接受治疗的SPTP及PNET作为研究对象,分为SPTP组(31例)及PNET组(86例),使用西门子双源CT进行扫描,比较两组CT扫描影像学特征,对该法鉴别SPTP及PNET的价值进行评估。结果SPTP组钙化、浮云征比例明显高于PNET组,动脉期及静脉期增幅强化绝对值明显低于PNET组(P<0.05);SPTP组动脉期增强峰值时相、壁结节样强化比例明显低于PNET组,环形强化比例明显高于PNET组(P<0.05);SPTP组静脉期碘浓度(IC)、标准化碘浓度NIC明显高于PNET组(P<0.05);静脉期IC、静脉期NIC对SPTP与PNET鉴别的AUC值分别为0.723、0.807。结论STPT、PNET患者DSCT扫描影像学特征存在一定差异,利用DSCT对二者鉴别具有良好的应用价值。Objective To explore the differential value of Siemens dual-source computer tomography(DSCT)for solid pseudopapillary tumor of the pancreas(SPTP)and pancreatic neuroendocrine tumors(PNET).Methods The patients underwent SPTP or PNET were enrolled as the research objects.And they were divided into SPTP group(31 cases)and PNET group(86 cases).All underwent Siemens DSCT scans.The imaging features of CT scan were compared between the two groups.The differential value of the method for SPTP and PNET was evaluated.Results The proportions of calcification and cloud signs in SPTP group were significantly higher than those in PNET group,while absolute value of augmentation enhancement in arterial phase and venous phase was significantly lower than that in PNET group(P<0.05).The enhancement peak time phase in arterial phase and proportion of wall-nodule enhancement in SPTP group were significantly lower than those in PNET group,while proportion of ring-enhancement was significantly higher than that in PNET group(P<0.05).The iodine concentration(IC)and normalized iodine concentration(NIC)in venous phase of SPTP group were significantly higher than those of PNET group(P<0.05).AUC values of IC and NIC in venous phase for distinguishing SPTP from PNET were 0.723 and 0.807,respectively.Conclusion There are certain differences in the imaging features of DSCT scan between patients with STPT and those with PNET.DSCT is of good differential value for SPTP and PNET.
