摘　　要：目的系统评价针灸对胃癌患者化疗期间不良反应的临床疗效。方法通过计算机检索中文期刊全文数据库(CNKI)、中国生物医学文献数据库(CBM)、维普期刊数据库(VIP)、万方医学网(WANFANG)和PubMed、MEDLINE、Web of Science数据库建库至2021年1月的针灸治疗胃癌患者化疗期间不良反应的临床随机对照试验研究文献,运用RevMan5.3软件进行Meta分析。结果筛选出文献350篇,根据文献纳入排除标准,排除343篇,最终纳入7篇临床研究,共计484例患者,其中治疗组242例,对照组242例。治疗组的临床症状改善情况[OR=2.05,95%CI(1.10,3.83),P<0.05]、KPS评分[OR=3.54,95%CI(1.86,6.74),P<0.05]优于对照组。治疗组在改善免疫功能、骨髓抑制、肝肾功能损害、恶心呕吐等方面疗效均优于对照组,差异具有统计学意义(P<0.05);CD8+敏感性分析结果显示本次CD8+分析的结果较为稳定。结论针灸是治疗胃癌患者化疗期间不良反应的有效方法,能够明显改善化疗期间的不良反应,提高患者的生活质量。Objective To systematically evaluate the clinical efficacy of acupuncture and moxibustion on adverse reactions in patients with gastric cancer during chemotherapy.Methods The clinical randomized controlled trials(RCTs)of acupuncture and moxibustion on adverse reactions in patients with gastric cancer during chemotherapy were retrieved by computer from the Chinese databases of CNKI,Chinese Biomedical Literature Database(CBM),VIP Journal Database(VIP),Wanfang Medical Network(WANFANG)and oversea databases of PubMed,MEDLINE,Web of Science from the database establishment day to January 2021.Meta-analysis was carried out by using ReVman5.3 software.Results:A total of 350 RCTs were screened out.According to the inclusion and exclusion criteria,343 literature were excluded,and finally 7 clinical studies with a total of 484 patients were included,242 cases in the treatment group and 242 cases in the control group.The improvement of clinical symptoms[OR=2.05,95%CI(1.10,3.83),P<0.05]and KPS scores[OR=3.54,95%CI(1.86,6.74),P<0.05]in the treatment group was superior to that in the control group.The treatment group has stronger effect on improving patients’immune function,myelosuppression,liver and kidney function damage,nausea and vomiting than the control group,and the differences were statistically significant(P<0.05).The results of sensitivity analysis showed that the CD8 + analysis results were relatively stable. Conclusion Acupuncture is effective in the treatment of adverse reactions in patients with gastric cancer during chemotherapy, which can significantly improve the adverse reactions during chemotherapy and improve the quality of life of patients.
