营养支持联合适形调强放疗治疗全梗阻性食管癌的效果及对CyclinD1 p21和p27表达的影响

摘　　要：目的探讨全梗阻性食管癌患者采用营养支持联合适形调强放疗的效果及对细胞周期蛋白D1(CyclinD1)、p21和p27表达的影响。方法选取2012年3月—2019年12月盘锦市中心医院收治的全梗阻性食管癌患者87例,按照治疗方式的差异分为观察组41例和对照组46例,所有患者均接受适形调强放疗,观察组在实施适形调强放疗前进行营养支持干预。比较分析两组的临床疗效及治疗前后CyclinD1、p21和p27表达情况。结果观察组进食症状改善总有效率为90.24%,高于对照组的70.21%,差异有统计学意义(χ^(2)=5.400,P=0.020)。观察组近期梗阻缓解总有效率为92.68%,高于对照组的76.60%,差异有统计学意义(χ^(2)=4.236,P=0.040)。治疗后观察组CyclinD1表达阳性率低于对照组,p21和p27表达阳性率高于对照组,差异均有统计学意义(P值均<0.05)。结论营养支持联合适形调强放疗可进一步改善患者的进食和梗阻情况,并且能够下调CyclinD1表达,上调p21和p27表达。Objective To investigate the efficacy of nutritional support combined with conformal intensity modulated radiotherapy(IMRT)on patients with total obstructive esophageal cancer,and to analyze the expression of cyclin D1,p21 and p27.Methods A total of 87 patients with total obstructive esophageal cancer admitted to Panjin Central Hospital from March 2012 to December 2019 were included in this study.The patients were divided into observation group(n=41)and control group(n=46)according to the treatment methods.All subjects received IMRT,and the observation group received nutritional support intervention in addition to IMRT.The clinical efficacy and the expression of cyclin D1,p21 and p27 before and after treatment were analyzed between the two groups.Results The total effective rate of eating symptom improvement in observation group was significantly higher than that in control group(P<0.05).The total effective rate of alleviating obstruction in observation group was significantly higher than that in control group(P<0.05).After treatment,the positive rate of CyclinD1 expression in the observation group was significantly lower than that in the control group,while the positive rate of p21 and p27 expression was significantly higher than that in the control group(all P<0.05).Conclusion Nutritional support combined with IMRT can significantly improve the symptoms of eating and obstruction,and improve the nutritional status of patients,down-regulate the expression of CyclinD1,and up-regulate the expression of p21 and p27.
