摘 要:目的探讨急性缺血性脑卒中(AIS)患者脑组织微循环的磁共振灌注加权成像(MR-PWI)特点。方法对74例AIS患者进行MR-PWI检查,记录局部脑血流量(rCBF)、局部脑血容量(rCBV)、平均通过时间(MTT)和达峰值时间(TTP)。分析AIS患者MR-PWI各参数的异常情况和脑组织微循环的特点。结果AIS患者MR-PWI各参数异常率从高至低分别为TTP、MTT、rCBF、rCBV。74例患者中有3例为过度灌注,其余71例患者中有22例为缺血期(30.99%)、2例为代偿期(2.82%)、18例为失代偿期(25.35%)、29例为梗死期(40.85%)。结论MR-PWI检查中TTP比较敏感,临床上可以根据MR-PWI判断AIS患者不同时期的脑组织损伤程度,实现个体化评估,指导临床精准治疗。Objective To explore the magnetic resonance perfusion-weighted imaging(MR-PWI) signatures of cerebral microcirculation in patients with acute ischemic stroke(AIS).Methods Seventy-four AIS patients underwent MR-PWI examination to record their regional cerebral blood flow(rCBF),regional cerebral blood volume(rCBV),mean transit time(MTT),and time to peak(TTP).The abnormality of various parameters of MR-PWI and the characteristics of cerebral microcirculation were analyzed in AIS patients.Results Among AIS patients,the abnormal rates of various parameters of MR-PWI decreased in the sequence of TTP,MTT,rCBF,and rCBV.Among the 74 patients,three cases had hyperperfusion,and the remaining 71 cases included 22(30.99%) cases in the ischemia stage,two(2.82%) cases in the compensatory stage,18(25.35%) cases in the decompensated stage,and 29(40.85%) cases in the infarction stage.Conclusion TTP is more sensitive in MR-PWI examination,clinically,the severity of brain tissue injury in different stages can be identified in AIS patients based on MR-PWI,thus achieving individualized evaluation and guiding clinical precise treatment.
