腰围身高比值与急性胰腺炎严重程度的关系分析

摘　　要：目的研究腰围身高比值与急性胰腺炎严重程度的关系。方法选取2019年8月至2020年8月本院收治的120例急性胰腺炎患者作为研究对象。比较不同严重程度患者的一般资料、人体测量学指标,分析影响重症急性胰腺炎的危险因素。结果120例患者中,轻症36例(30.00%),中度重症56例(46.67%),重症28例(23.33%)。不同严重程度患者的一般资料比较,差异无统计学意义(P>0.05)。轻症患者的腰围、体重、腰围身高比值、身体质量指数均低于中度重症与重症患者,且中度重症患者低于重症患者(P<0.05)。多因素Logistic回归分析结果显示,腰围身高比值、呼吸频率、白蛋白、APACHEⅡ评分、初发是发生重症急性胰腺炎的独立危险因素(P<0.05)。且腰围身高比值的OR(95%CI)高于其他因素,提示腰围身高比值与重症急性胰腺炎的关系最密切。结论腰围身高比值与急性胰腺炎严重程度存在密切关系,可作为早期预测指标。Objective To study the relationship between waist-to-height ratio and severity of acute pancreatitis.Methods A total of 120 patients with acute pancreatitis treated in our hospital from August 2019 to August 2020 were selected as the research objects.The general data and anthropometric indexes of patients with different severity were compared,and the risk factors of severe acute pancreatitis was analyzed.Results Among the 120 patients,36 cases(30.00%)were mild,56 cases(46.67%)were moderate severe,and 28 cases(23.33%)were severe.There were no significant differences in the general data of patients with different severity(P>0.05).The waistline,weight,waist-to-height ratio and body mass index of mild patients were lower than those of moderate severe and severe patients,and those of moderate severe patients were lower than the severe patients(P<0.05).Multivariate Logistic regression analysis showed that waist-to-height ratio,respiration rate,albumin,APACHEⅡscore and initial onset were independent risk factors for severe acute pancreatitis(P<0.05).The OR(95%CI)of waist-to-height ratio was higher than that of other factors,suggesting that waist-to-height ratio was most closely related to severe acute pancreatitis.Conclusion Waist-to-height ratio is closely related to the severity of acute pancreatitis,which can be used as an early predictor.
