靴形曲联合橡皮链关闭拔牙间隙的临床疗效分析

摘　　要：目的分析靴形曲联合橡皮链关闭拔牙间隙的临床疗效。方法回顾性分析2018年12月至2019年7月在宁德师范学院附属宁德市医院口腔正畸科诊治的80例错[牙合]畸形患者的临床资料,根据治疗方法不同分为对照组和研究组,各40例。对照组采用橡皮链关闭拔牙间隙,研究组采用靴形曲联合橡皮链关闭拔牙间隙。比较两组治疗后的矫治结果以及治疗前后两组上下颌牙齿、骨骼、软组织和前牙覆盖(OJ)、前牙覆[牙合](OB)值变化情况。结果两组平均治疗时间15个月,所有患者均矫治成功,磨牙、尖牙为中性关系,OJ、OB值正常,尖窝锁结关系良好。间隙完全关闭,在关闭前牙间隙过程中未出现不良反应。治疗前后U1-V距、L1-V距、U1-NA角、U1-NA距、L1-NB角、L1-NB距以及上唇齿高、下唇齿高、上唇-E、下唇-E、OJ值、OB值的主效应差异有统计学意义(P<0.01);不考虑测量时间,两组间U1-V距、L1-V距、U1-NA角、U1-NA距、L1-NB角、L1-NB距以及上唇齿高、下唇齿高、上唇-E、下唇-E、OJ值、OB值的主效应差异有统计学意义(P<0.01);U1-V距、L1-V距、U1-NA角、U1-NA距、L1-NB角、L1-NB距以及上唇齿高、下唇齿高、上唇-E、下唇-E、OJ值、OB值的组间与时点间存在交互作用(P<0.01);治疗后两组U1-V距、L1-V距、U1-NA角、U1-NA距、L1-NB角、L1-NB距以及上唇齿高、下唇齿高、上唇-E、下唇-E、OJ值、OB值均降低,研究组变化较明显。结论靴形曲联合橡皮链关闭拔牙间隙治疗错[牙合]畸形的临床疗效显著,可有效关闭拔牙间隙。Objective To analyze the clinical effect of the shoe-shaped loop combined rubber chain in closing the tooth extraction gap.Methods A retrospective analysis of 80 cases of malocclusion diagnosed and treated in the Department of Orthodontics,Ningde Hospital Affiliated to Ningde Normal University from Dec.2018 to Jul.2019 was done.According to different treatment methods,they were divided into a control group(rubber chain to close the tooth extraction gap)and a research group(shoe-shaped loop combined with rubber chain to close the tooth extraction gap),40 cases each.The correction results of the two groups after treatment were compared,and the changes in the maxillary and mandibular teeth,bones,soft tissues,and the values of overjet(OJ)and over-bite(OB)of the two groups before and after treatment were observed and recorded.Results The average treatment time of both groups was 15 months.All cases were successfully corrected.The molar and canine had a neutral relationship,the OJ and OB values were normal,and the apical fossa locking was good.The space was completely closed,and no adverse reactions occurred during the closing of the anterior space.There were significant differences in the main effects of U1-V distance,L1-V distance,U1-NA angle,U1-NA distance,L1/NB angle,L1-NB distance,upper lip tooth height,lower lip tooth height,upper lip-E,lower lip-E,OJ and OB values before and after treatment(P<0.01).Regardless of measurement time,the main effects of the above indexes had statistically significant differences between the two groups(P<0.01);all of the above indexes had interactions between groups and time points(P<0.01).After treatment,all of the above indexes decreased in both groups,and the changes were more obvious in the research group.Conclusion The application of shoe-shaped loop combined with rubber chain to close the tooth extraction gap has significant clinical effect in treating malformations and deformities,which can effectively close the extraction gap.
