育阴潜阳汤对抗精神病药所致帕金森综合征患者认知功能的影响

摘　　要：目的探讨育阴潜阳汤治疗抗精神病药所致帕金森综合征的疗效及对患者认知功能的影响。方法应用随机、双盲对照研究方法,将2017年10月1日—2019年12月31日在上海市精神卫生中心治疗的120例抗精神病药物所致帕金森综合征患者随机分为研究组60例和对照组60例,分别予以育阴潜阳汤+盐酸苯海索片安慰剂、盐酸苯海索片+育阴潜阳汤安慰剂治疗,治疗周期均为12周。比较2组基线及治疗第2,4,8,12周末不自主运动量表(AIMS)评分、自拟中医证候量表评分、临床疗效总评量表(CGI)评分和基线、治疗第12周末重复性成套神经心理状态评定量表(RBANS)评分。结果2组患者AIMS评分、中医证候量表评分、CGI评分组间差异均无统计学意义(P均>0.05),即不存在分组效应;治疗后不同时间点AIMS评分、中医证候量表评分、CGI评分差异均有统计学意义(P均<0.05),即存在时间效应;2组患者AIMS评分、中医证候量表评分、CGI评分均随治疗时间的延长逐渐降低。治疗第12周末,研究组患者即刻记忆、言语功能、注意、延时记忆评分和总分均明显高于治疗前及对照组(P均<0.05)。结论育阴潜阳汤治疗抗精神病药物所致帕金森综合征疗效较好,并可改善患者的认知功能。Objective It is to investigate the efficacy of Yuyin Qianyang Decoction in the treatment of Parkinson’s syndrome caused by antipsychotics and its effect on cognitive function.Methods A total of 120 patients with Parkinson’s syndrome caused by antipsychotic drugs treated at the Shanghai Mental Health Center from October 1,2017 to December 31,2019 were randomly divided into a study group and a control group by a randomized,double-blind,controlled study method,60 cases each group.The study group was treated with Yuyin Qianyang Decoction+trihexyphenidyl hydrochloride tablets placebo,and the control group was treated with trihexyphenid hydrochloride tablets+Yuyin Qianyang Decoction placebo,both groups were treated for 12 weeks.The scores of abnormal involuntary movement scale(AIMS),scores of self-developed TCM syndrome scale,scores of clinical global impression(CGI)at baseline and after treatment for 2,4,8,and 12 weeks,and the scores of repeatable battery for the assessment of neuropsychological status(RBANS)at baseline and after treatment for 12 weeks were compared between the two groups.Results There was no significant difference in AIMS score,TCM syndrome scale score,and CGI score between the two groups(P>0.05),that was,there was no grouping effect;the difference in AIMS score,TCM syndrome scale score,and CGI score at different time points after treatment was statistically significant(P<0.05),that was,there was a time effect;the AIMS score,TCM syndrome scale score,and CGI score of the two groups of patients all gradually decreased with the extension of treatment time.At the 12th weekend of treatment,the scores and total scores of immediate memory,speech function,attention,and delayed memory of the study group were significantly higher than those before treatment and the control group(all P<0.05).Conclusion Yuyin Qianyang Decoction is effective in the treatment of Parkinson’s syndrome caused by antipsychotics,and can improve the cognitive function of patients.
