健脾化痰方联合FOLFOX6方案治疗结直肠癌肝转移的临床疗效及安全性

摘　　要：目的探讨健脾化痰方联合FOLFOX6方案治疗结直肠癌肝转移的疗效及安全性。方法筛选南京中医药大学第二附属医院2018年2月—2020年2月收治的结直肠癌肝转移患者60例,参照随机数字表法分为2组各30例。对照组给予FOLFOX6方案系统治疗;观察组在FOLFOX6方案系统治疗基础上给予健脾化痰方内服,每天1剂。2组疗程均为12周。统计2组实体瘤近期疗效、体力状况疗效,观察治疗前后2组患者脾虚毒蕴证主症评分及血清癌胚抗原(CEA)、糖类抗原199(CA199)水平变化,比较2组毒副反应发生情况。结果观察组实体瘤近期总有效率及体力状况改善率均显著高于对照组(P均<0.05);治疗后,观察组脾虚毒蕴证主症评分以及血清CEA、CA199水平显著低于对照组(P均<0.05);治疗期间,观察组白细胞减少、胃肠道反应发生率均显著低于对照组(P均<0.05)。结论健脾化痰方联合FOLFOX6方案治疗结直肠癌肝转移脾虚毒蕴证近期疗效更高,可进一步改善患者的体力,减轻中医症状,下调血清CEA、CA199水平,且能减少毒副反应。Objective It is to investigate the efficacy of and safety of Jianpi Huatan decoction combined with FOLFOX6 regimen in the treatment of liver metastasis of colorectal cancer.Methods Sixty patients with colorectal cancer and liver metastasis admitted to the Second Affiliated Hospital of Nanjing University of Traditional Chinese Medicine from February 2018 to February 2020 were selected and divided into 2 groups by the random number table method,30 cases in each group.The control group was treated with FOLFOX6 regimen.The observation group was given Jianpi Huatan Decoction by oral administration on the basis of FOLFOX6 system treatment,one dose per day.The course of treatment in both groups was 12 weeks.The short-term curative effect and physical status curative effect of the solid tumors were assessed in the two groups,the main symptom scores of spleen-deficiency and toxin accumulation and the changes in the levels of serum carcinoembryonic antigen(CEA)and carbohydrate antigen 199(CA199)were observed before and after treatment in the two groups,the occurrence of side effects were compared between the two groups.Results The short-term total effective rate and improvement rate of physical status of solid tumors in the observation group were significantly higher than those in the control group(all P<0.05);after treatment,the scores of the main symptoms of spleen deficiency and toxin accumulation and the levels of serum CEA and CA199 in the observation group were significantly lower than those in the control group(all P<0.05);during the treatment period,the incidence of leukopenia and gastrointestinal reactions in the observation group was significantly lower than that in the control group(all P<0.05).Conclusion Jianpi Huatan Decoction combined with FOLFOX6 has a better short-term curative effect in the treatment of liver metastasis of colorectal cancer with syndrome of spleen deficiency and toxic aggregating,which can further improve the patient’s physical strength,relieve the symptoms of traditional Chinese medicin
