摘 要:目的研究肺癌诊断及病理组织分型中行血清肿瘤标志物诊断的效果。方法选取2018年9月至2020年9月九江市第一人民医院收治的50例肺癌患者作为观察组,另选取同期收治的50例健康体检者作为对照组,两组患者行血清肿瘤标志物[癌胚抗原(CEA)、糖类抗原125(CA125)、糖类抗原199(CA199)、神经特异性烯醇化酶(NSE)、细胞角蛋白19片段(Cyfra21-1)]诊断,对诊断结果比较分析。结果观察组患者的CEA、CA125、CA199、NSE及Cyfra21-1高于对照组,差异有统计学意义(P<0.05)。腺癌的CEA、CA125、CA199、NSE及Cyfra21-1的敏感性低于鳞癌及小细胞肺癌,差异有统计学意义(P<0.05)。鳞癌、腺癌、小细胞癌两两比较,差异均有统计学意义(P<0.05)。诊断肺癌时,灵敏性从高到低依次为CEA、CA125、CA199、NSE及Cyfra21-1,特异性从高到低依次为CEA、CA199、CA125、Cyfra21-1及NSE。结论肺癌的病理组织分型与CEA、CA125、CA199、NSE及Cyfra21-1水平有关,其早期诊断肺癌的敏感性及特异性较高,可早期确诊病情、改善患者预后,值得推广。Objective To study the diagnostic effect of serum tumor markers in the diagnosis of lung cancer and histopathological classification.Methods A total of 50 lung cancer patients admitted to the First People′s Hospital of Jiujiang City from September 2018 to September 2020 were selected as the observation group,and another 50 healthy individuals during the same period were selected as the control group.Serum tumor markers(carcino-embryonic antigen[CEA],carbohydrate antigen 125[CA125],carbohydrate antigen 199[CA199],neurospecific enolase[NSE]and cytokeratin 19 fragment[Cyfra21-1])were diagnosed in the two groups,and the diagnostic results were compared and analyzed.Results CEA,CA125,CA199,NSE and Cyfra21-1 in observation group were higher than those in control group,and the differences were statistically significant(P<0.05).The sensitivity of CEA,CA125,CA199,NSE and Cyfra21-1 in adenocarcinoma was lower than that in squamous cell carcinoma and small cell lung cancer,and the difference was statistically significant(P<0.05).There were significant differences in squamous cell carcinoma,adenocarcinoma and small cell carcinoma(P<0.05).In the diagnosis of lung cancer,the sensitivity from high to low was CEA,CA125,CA199,NSE and Cyfra21-1,and the specificity from high to low was CEA,CA199,CA125,Cyfra21-1 and NSE.Conclusion The histopathological classification of lung cancer is related to the levels of CEA,CA125,CA199,NSE and Cyfra21-1.Its sensitivity and specificity for early diagnosis of lung cancer are relatively high.It can diagnose the disease early and improve the prognosis of patients.It is worthy of promotion.
