艾司奥美拉唑两种用药方案治疗消化性溃疡并出血临床观察

摘　　要：目的:探讨在治疗消化性溃疡并出血患者中,艾司奥美拉唑静脉注射以及艾司奥美拉唑“808方案”的临床效果及其安全性,为临床合理的治疗方案提供参考依据。方法:筛选出2019年2月至2021年4月因呕血和/或黑便而在萍乡市人民医院就诊并确诊为消化性溃疡并出血患者83例,随机分成两组,其中对照组40例,采用艾司奥美拉唑40 mg+0.9%氯化钠注射液静脉注射。观察组采用艾司奥美拉唑“808方案”泵入治疗:首次艾司奥美拉唑80 mg静脉注射,之后以8 mg/h(艾司奥美拉唑80 mg+0.9%氯化钠注射液40 mL)微量泵泵入(泵速4 mL/h)。均为每8~12小时一次,使用72 h。对两组治疗效果进行评价,对两组患者平均住院时间、平均止血时间、出血量、血红蛋白水平、输血量、住院费用、不良反应进行记录。结果:相比于对照组,观察组总有效率更明显,平均住院时间、止血时间明显缩短,出血量明显减少,血红蛋白水平明显更高,输血量明显减少,住院费用明显降低,未增加不良反应发生率(P<0.05)。结论:相比艾司奥美拉唑静脉注射而言,采用艾司奥美拉唑“808方案”泵入治疗消化性溃疡并出血效果更显著,且具有同样高的安全价值。Objective:To explore the clinical effects and safety of esomeprazole intravenous drip or bolus injection and esomeprazole 808 regimen in the treatment of patients with peptic ulcer and bleeding,and provide a reference for clinically reasonable treatment plans.Methods:A total of 83 patients with peptic ulcer and bleeding who were diagnosed with peptic ulcer and bleeding from February 2019 to April 2021 in Pingxiang People's Hospital due to hematemesis and/or melena were selected.Someprazole was injected intravenously or as a bolus;43 cases in the observation group were treated with Esomeprazole 808 regimen.The treatment effects of the two groups were evaluated,and the average length of hospitalization、average hemostasis time、bleeding volume、hemoglobin level、blood transfusion volume、hospitalization expensesand adverse reactions of the two groups were recorded.Results:Compared with the control group,the total effective rate was more obvious in the observation group,P<0.05.Compared with the control group,the observation group's average hospital stay,hemostasis time were significantly shorter,and the amount of bleeding was significantly less,significantly higher hemoglobin levels,significantly less blood transfusion,and significantly less hospitalization costs,the incidence of adverse reactions in the observation group was not significant.Conclusion:Compared with intravenous drip or bolus injection of esomeprazole,the use of esomeprazole 808 regimen in the treatment of peptic ulcer and bleeding is more effective and has a same safety value.
