胃部分切除手术治疗早期胃癌的临床效果研究

摘　　要：目的探讨胃部分切除手术在早期胃癌治疗中的应用效果。方法120例早期胃癌患者,基于随机数字表法分为对照组与观察组,每组60例。对照组采用常规远端胃切除术治疗,观察组采用胃部分切除手术治疗。比较两组临床指标,胃排空时间和体质量指数(BMI),并发症发生情况。结果观察组患者术后排气时间、肠道功能恢复时间和手术时间均明显短于对照组,术中出血量少于对照组,差异具有统计学意义(P<0.05)。观察组患者胃排空时间(16.48±3.25)s短于对照组的(37.60±3.83)s,BMI(21.53±2.44)kg/m^(2)高于对照组的(16.36±2.02)kg/m^(2),差异具有统计学意义(P<0.05)。观察组患者的并发症发生率3.33%低于对照组的15.00%,差异具有统计学意义(χ^(2)=4.904,P=0.027<0.05)。结论胃部分切口手术在早期胃癌治疗中具有较高的应用价值,除加快术后排气时间和胃排空时间外,有助于降低并发症发生率,值得临床进一步采纳与推广。Objective To discuss the practical effect of partial gastrectomy in the treatment of early gastric cancer.Methods A total of 120 patients with early gastric cancer were divided into the control group and the observation group according to random numerical table,with 60 cases in each group.The control group was treated with conventional distal gastrectomy,and the observation group was treated with partial gastrectomy.The clinical indicators,gastric emptying time,body mass index(BMI),and complications were compared between the two groups.Results The postoperative exhaust time,intestinal function recovery time and operation time of the observation group were significantly shorter than those of the control group,and the intraoperative blood loss was less than that of the control group.All the differences were statistically significant(P<0.05).The gastric emptying time(16.48±3.25)s of the observation group was significantly shorter than(37.60±3.83)s of the control group,and BMI(21.53±2.44)kg/m^(2) was higher than(16.36±2.02)kg/m^(2) of the control group.All the differences were statistically significant(P<0.05).The complication rate of the observation group was 3.33%,which was significantly lower than 15.00%of the control group,and the difference was statistically significant(χ^(2)=4.904,P=0.027<0.05).Conclusion Partial gastrectomy has high practical value in the treatment of early gastric cancer.It is helpful to reduce the incidence of complications in addition to speeding up the postoperative exhaust time and gastric emptying time,which is worthy of further clinical adoption and promotion.
