摘 要:目的探讨脑胶质瘤磁共振扩散峰度成像(DKI)与分子标记物缺氧诱导因子1α(HIF-1α)和基质金属蛋白酶-2(MMP-2)表达的关系。方法38例脑胶质瘤患者术前行DKI扫描。使用后处理软件测得肿瘤实质各向异性分数(FA)、平均扩散系数(MD)、以及峰度参数(轴向弥散峰度(Ka)、径向弥散峰度(Kr)及平均扩散峰度(MK))值。采用免疫组化技术评估HIF-1α和MMP-2的表达。Pearson相关性分析,用于评估DKI各参数值与HIF-1α、MMP-2表达量的关系。结果HIF-1α的表达水平与各峰度参数值呈显著的正相关关系(MK:r=0.593,P=0.001;Ka:r=0.609,P=0.001;Kr:r=0.573,P=0.002)。MMP-2的表达水平与各峰度参数值也呈显著的正相关关系(MK:r=0.576,P=0.002;Ka:r=0.598,P=0.001;Kr:r=0.568,P=0.002)。结论DKI这一无创技术所获得的峰度参数可以间接反映HIF-1α和MMP-2的表达情况,可以作为一种潜在的影像学标记来评估胶质瘤的缺氧环境和侵袭特性。Objective To investigate the association of diffusional kurtosis imaging(DKI)and the molecular markers of hypoxia inducible factor 1α(HIF-1α)and matrix metalloproteinase-2(MMP-2)in gliomas.Methods 38 patients with pathologically diagnosed gliomas underwent DKI examination.The values of fractional anisotropy(FA),mean diffusivity(MD),and kurtosis parameters(axial kurtosis(Ka),radial kurtosis(Kr),and mean kurtosis(MK))in the solid parts of the tumors were calculated using the post-processing software.The expressions of HIF-1αand MMP-2 were measured by immunohistochemistry.The pearson correlation analysis test was used to assess the relationship between the HIF-1αand MMP-2 expression and the DKI metrics.Results The level of HIF-1αexpression was significantly correlated with kurtosis parameters values(MK r=0.593,P=0.001;Ka r=0.609,P=0.001;Kr r=0.573,P=0.002).There was a significant positive correlation between the kurtosis parameters values and MMP-2expression as well(MK r=0.576,P=0.002;Ka r=0.598,P=0.001;Kr r=0.568,P=0.002).Conclusion The kurtosis parameters obtained by the non-invasive technology of DKI can indirectly reflect the expression of HIF-1αand MMP-2,and would be used as a potential imaging marker to evaluate the hypoxic environment and infiltrating characteristics of gliomas.
