摘 要:目的探讨少见部位少突胶质细胞肿瘤的影像学特征,以提高诊断正确率。方法选取我院经病理证实的10例少见部位少突胶质细胞肿瘤的术前影像学检查资料,对其影像学特征进行回顾性分析。结果10例中WHO Ⅱ级4例,WHO Ⅲ级6例。病变4例位于第三脑室及侧脑室,3例位于丘脑,鞍上、胼胝体及小脑各1例。大多数病变内部存在囊性变(9/10),边缘与周围脑组织分界不清。肿瘤实质部分CT呈稍高密度,MRI呈稍长T_(1)稍长T_(2)信号,囊性部分呈脑脊液样密度或信号。增强扫描实性成分呈中度-明显强化、囊性成分不强化。MRS有助于鉴别诊断。结论CT和MRI的联合应用能够全面显示少突胶质细胞瘤的影像学特点,缩小鉴别诊断范围,提高影像诊断正确率。Objective To improve the diagnostic accuracy for oligodendrocyte tumors in rare locations by exploring the imaging features of the diseases.Methods 10 pathologically confirmed oligodendroglioma in rare sites were collected in our hospital before surgery,then the imaging characteristics were retrospectively analyzed.Results In all the cases,4 lesions were diagnosed as WHO grade Ⅱ and 6 were WHO grade Ⅲ.There were 4 cases in the third ventricle and lateral ventricle,3 cases in thalamus,and other 3 cases in saddle area,corpus callosum,and cerebellum,respectively.Cystic degeneration was detected in most of the tumors(9/10)without clear demarcation between the lesions and adjacent brain tissue.The parenchyma of the tumors showed slight hypodensity on CT,and hypo-signal intensity on MRI T_(1)WI with hyper-signal intensity on T_(2)WI.There was moderate to remarkable enhancement in the parenchymal portion of the tumors.The cystic part of the lesions exhibited CT density and MRI signal intensity similar to cerebral spinal fluid without contrast enhancement.MR spectroscopy can provide information for differential diagnosis.Conclusion Imaging characteristics of oligodendrocyte tumors in rare sites can be demonstrated by CT and MRI from different angles,which can effectively narrow the differential possibilities and improve the diagnostic accuracy of the disease.
