下颌近中阻生第三磨牙对邻牙牙周健康水平的影响

摘　　要：目的分析不同类型近中阻生第三磨牙对邻近第二磨牙牙周健康水平的影响,评价拔除第三磨牙对第二磨牙牙周健康维护的作用。方法67例近中阻生第三磨牙,按阻生类型纳入高位阻生组(25例)、中位阻生组(22例)以及低位阻生组(20例)。记录受试者基本信息及牙周基本状况后拔除第三磨牙。记录并分析治疗前及术后4周复诊时受试者菌斑指数(PLI)、牙周探诊深度(PD)、牙龈指数(GI)、出血指数(BI)、临床附着丧失(CAL)。结果各类第三磨牙阻生均对邻牙牙周健康水平有影响,与高位阻生相比,低位阻生组第二磨牙PD、BI、GI、CAL等指标显著下降。当第三磨牙拔除后,第二磨牙各项牙周指标均有好转。结论近中阻生第三磨牙可导致邻牙牙周健康水平下降,拔除第三磨牙可有效改善这一状况。Objective To analyze the effects of different types of mesial impacted third molars on the periodontal health of adjacent second molars,and to evaluate the effect of third molar extraction on the periodontal health maintenance of second molars.Methods 67 cases of mesial impacted third molars were divided into high impacted group(25 cases),middle impacted group(22 cases)and low impacted group(20 cases).After recording the basic information and periodontal status of the subjects,the third molar was removed.Plaque index(PLI),periodontal probing depth(PD),gingival index(GI),bleeding index(BI)and clinical attachment loss(CAL)were recorded and analyzed.Results All kinds of third molar impaction had an impact on the periodontal health of adjacent teeth.Compared with high impaction,the indexes of PD,Bi,GI and Cal of second molar in low impaction group decreased significantly.When the third molar was extracted,the periodontal indexes of the second molar were improved.Conclusion Mesial impacted third molars can reduce the periodontal health of adjacent teeth,and extraction of third molars can effectively improve this situation.
