高度近视准分子激光原位角膜磨镶术后对比敏感度变化的动态研究

摘　　要：目的探讨光学切削直径与暗视状态瞳孔直径大小关系对患者对比敏感度的影响。方法收集2019年6月至2020年6月在宁夏医科大学总医院接受准分子角膜磨镶术(LASIK)治疗的高度近视患者70例(140眼)。光学切削直径大于或等于暗视状态瞳孔直径的患者为A组,光学切削直径小于暗视状态瞳孔直径的患者为B组,各35例。对70例患者于LASIK术前和术后1周、1个月、3个月、6个月检测明视觉对比敏感度(photopic CS)、中间视觉敏感度(mesopic CS)及眩光敏感度(GS)。结果两组患者photopic CS在术后1周时降低,术后3个月恢复至术前水平,术后6个月高于术前(P均<0.05),各时间点两组间比较差异均无统计学意义(P均>0.05);两组患者mesopic CS、GS在术后1周时各等级未通过率均较术前增加(P均<0.05);两组患者mesopic CS以及A组患者的GS在术后3个月时未通过率较术前降低(P均<0.05),而B组患者GS的未通过率在术后6个月仍高于术前水平(P<0.05);不同时间B组各等级未通过率均高于A组(P均<0.05)。结论光学切削直径与暗视状态瞳孔直径大小关系对CS影响不大,但当光学切削直径小于暗视状态瞳孔直径时患者中间视觉对比敏感度下降明显,眩光时更为显著。Objective To explore the influence of the relationship between optical ablation diameter and scotopic pupil diameter on contrast sensitivity.Methods Seventy cases(140 eyes)with high myopia were collected.The patients whose optical ablation diameter was larger than scotopic pupil diameter were in group A,while those whose optical ablation diameter was smaller than scotopic pupil diameter were in group B,with 35 patients in each group.The photopic CS,mesopic CS and GS were measured for 70 patients before LASIK and 1 week,1 month,3 months and 6 months postoperatively.Results The photopic CS of two groups decreased significantly at 1 week after operation,returned to the preoperative level at 3 months after operation,and increased at 6 months after operation(P all<0.05),but there was no statistical difference between the two groups at different time(P all>0.05);The rate of failing to mesopic CS and of the two groups at 1 week after operation were higher than those before operation(P all<0.05);At 3 months after operation the rate of failing to the mesopic CS of two groups and the GS of group were lower than preoperative(P all<0.05);The rate of failing to the GS of group B was still higher than preoperative at 6 months after operation(P<0.05);The rate of failing to each grade in group B was higher than that in group A(P all<0.05).Conclusion The relationship between optical ablation diameter and scotopic pupil diameter had little effect on the photopic CS,but when the optical ablation diameter is smaller than the scotopic pupil diameter,the mesopic CS decreases significantly,especially in glare.
