摘　　要：目的:分析早期胃癌内镜黏膜下剥离术(ESD)后追加胃切除术的影响因素。方法:回顾性分析2012年1月-2020年12月行ESD并追加外科胃切除术早期胃癌患者40例的临床资料,比较ESD后基底或切缘彻底性的影响因素。结果:早期胃癌ESD后追加胃切除术的影响因素包括基底或切缘阳性、黏膜下层侵犯、脉管侵犯及分化类型差。基底或切缘彻底性与浸润深度有密切关系(P<0.01)。结论:基底或切缘阳性、黏膜下层侵犯、脉管侵犯及分化类型差是影响非治愈性ESD后需追加胃切除术的因素。Objective:To analyze the influencing factors of additional gastrectomy after endoscopic submucosal dissection(ESD)for early gastric cancer.Methods:From January 2012 to December 2020,the clinical data of 40 patients with early gastric cancer who underwent ESD and additional surgical gastrectomy were analyzed retrospectively.We compared the factors influencing the thoroughness of the basal or margin after ESD.Results:The influencing factors of additional gastrectomy after ESD for early gastric cancer were positive basal or resection margin,submucosal invasion,vascular invasion and poor differentiation.The thoroughness of the basal or incisal margin is closely related to the depth of infiltration(P<0.01).Conclusion:Positive basal or resection margin,submucosal invasion,vascular invasion and poor differentiation are factors that affect the need for additional gastrectomy after non-curable ESD.
