小脑幕脑膜瘤的手术治疗和手术入路选择效果分析

摘　　要：目的:探讨小脑幕脑膜瘤的手术治疗和手术入路选择。方法:2020年1月-2021年1月收治小脑幕脑膜瘤患者50例,均采取显微镜手术治疗,然后依据肿瘤生长的大小、部位、类型,选择适宜手术入路,评价手术切除的程度、功能状态、并发症情况。结果:术后患者切除程度:达到SimpsonⅠ级38例,Ⅱ级5例,Ⅲ级4例,Ⅳ级2例,Ⅴ级1例;患者术后KPS评分明显高于术前,差异有统计学意义(P<0.05);患者术后生活质量评分相较于术前提升,差异有统计学意义(P<0.05);术后患者出现颅内感染、肺部感染、硬膜外血肿各1例,无颞叶挫伤,并发症发生率为6.00%(3/50)。结论:针对小脑幕脑膜瘤,临床选择显微镜手术切除的疗效确切,可改善患者脑功能,同时安全性较高;另外依照肿瘤具体情况结合术者经验设计手术入路是手术成功的关键。Objective:To investigate the surgical treatment and surgical approach of tentorial meningioma.Methods:From January 2020 to January 2021,50 patients with tentorial meningioma were selected and treated with microsurgery,then,according to the size,location and type of tumor growth,we selected the appropriate surgical approach,and evaluated the degree of surgical resection,functional status and complications.Results:The degree of resection was Simpson gradeⅠin 38 cases,gradeⅡin 5 cases,gradeⅢin 4 cases,gradeⅣin 2 cases and gradeⅤin 1 case.The postoperative KPS score was significantly higher than the preoperative score,the difference was statistically significant(P<0.05).The postoperative quality of life score was improved compared with that before operation,the difference was statistically significant(P<0.05).There was 1 case of intracranial infection,1 case of pulmonary infection and 1 case of epidural hematoma,there was no temporal lobe contusion,the incidence of complications was 6.00%(3/50).Conclusion:For tentorial meningiomas,microsurgical resection is effective in clinic,it can improve the brain function of patients with high safety performance.In addition,designing the surgical approach according to the specific situation of the tumor and the experience of the operator is the key to the success of the operation.
