摘 要:目的:分析河南省2015至2019年李斯特菌病分子流行病学特征。方法:对河南省2015至2019年李斯特菌病感染专项监测中分离的60株单核细胞增生李斯特菌进行全基因组测序。分析其谱系、克隆群(CC型)、序列型、血清群。通过VFDB、BIGSdb-Lm和ResFinder数据库获得其毒力基因、生物杀灭剂和重金属抗性基因及耐药基因分布。采用核心基因组多位点序列(cgMLST)和脉冲场凝胶电泳(PFGE)分型方法进行分子分型。结果与结论:60株中65.0%来自于围生期病例,75.0%来自于血液标本。60株分属2个谱系(Ⅰ和Ⅱ),以谱系Ⅰ为优势谱系(68.3%);分属3个血清型(Ⅱb、Ⅱa和Ⅳb),Ⅱb为优势型(63.3%);分属13个CC型以及2个未分CC型(ST619和ST382),CC87为优势型(35.0%);分属18个序列型,ST87为优势型(35.0%)。所有菌株均携带毒力岛LIPI-1,26.7%菌株携带LIPI-3,45.0%菌株携带LIPI-4。所有菌株均不携带生物杀灭剂抗性基因(bcrABC、emrE、emrC、qacA、qacC和Tn6188 qac)和重金属抗性基因cadC,10.0%菌株携带重金属抗性基因cadA;28.3%菌株携带应激生存岛SSI-1,3.3%菌株携带SSI-2。60株均携带耐药基因fosX,3.3%菌株同时携带耐药基因fosX、mef(A)、msr(D)和tet(M),主要是大环内酯类和四环素类耐药基因。60株分属52种CL型,每种CL型包括1~5株菌,等位基因差异数0~1730个,CL0029、CL0014、CL0032和CL0033为优势型。60株共获得51种PFGE带型,每种带型包括1~4株菌,相似度为56.7%~100.0%,GX6A16.HA0030、GX6A16.HA0005、GX6A16.HA0023和GX6A16.HA0011为优势带型。Aim:To analyze the epidemiological characteristics of listeriosis in Henan from 2015 to 2019.Methods:Whole-genome sequencing was performed on 60 Listeria monocytogenes strains isolated from Henan during 2015 to 2019.The pedigree,clones(CC),sequence typing(ST),and serotype were analyzed.The virulence genes,biocide resistance genes and heavy metal resistance genes were obtained by VFDB,BIGSdb-Lm and ResFinder databases.Molecular typing was performed using core genomic multilocus sequence(cgMLST)and pulsed field gel electrophoresis(PFGE)typing methods.Results and Conclusion:Out of the 60 strains,65.0%were from perinatal cases,75.0%were from blood specimen.The 60 strains belonged to two lineages,pedigreeⅠwas dominant,accounting for 68.3%;serum group was mainlyⅡb,accounting for 63.3%;they were divided into 13 CC types and 2 unclassified CC types of ST619 and ST382,CC87 was the dominant type and accounting for 35.0%;they were divided into 18 ST types,and ST87 was the dominant type,accounting for 35.0%.All strains carried virulence island LIPI-1,26.7%strains carried LIPI-3,45.0%strains carried LIPI-4.All strains did not carry biocide resistance genes(bcrABC,emrE,emrC,qacA,qacC and Tn6188 qac)or heavy metal resistance gene cadC,10.0%strains carried heavy metal resistance gene cadA.28.3%strains had stress survival island SSI-1,3.3%strains had SSI-2.All strains carried resistance gene fosX,and 3.3%of the 60 strains carried resistance genes fosX+mef(A)+msr(D)+tet(M),mainly macrolide and tetracycline resistance genes.According to cgMLST analysis,60 strains belonged to 52 CL types,and each CL type included 1-5 strains with 0-1730 allele differences;CL0029,CL0014,CL0032 and CL0033 were the dominant CL types.According to PFGE typing,51 bands were obtained from 60 strains.Each band type included 1-4 strains with similarity of 56.7%-100.0%.The dominant bands were GX6A16.HA0030,GX6A16.HA0005,GX6A16.HA0023 and GX6A16.HA0011.
